Additional Job Description
MINIMUM QUALIFICATIONS :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
- National Certification by one of the recognized APRN Certification Centers.
- Licensure or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
- BLS Certification required with additional Advanced Life Support training at the discretion of the department.
- Fundamentals of Critical Care Support (FCCS) Certification required immediately upon hire (based on class availability).
EXPERIENCE:
- Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- ACGNP certification or ACNP preferred, but would consider FNP.
EXPERIENCE:
- Acute care, critical care experience preferred.
CORE DUTIES AND RESPONSIBILITIES:
- Manages comprehensive and individualized care for patients and families in the Inpatient/Critical care phase of hospitalization
- With the collaboration of Critical Care staff or by following established protocol and/or scope of practice and privileging guidelines, orders additional tests or consults to medically manage the patient.
- Performs, interprets and communicates results of diagnostic tests.
- Consults and communicates with Critical Care and inter-professional services regarding abnormal findings in the evaluations of patients.
- Develops a comprehensive individualized plan of care for patients that include teaching, establishing disposition/discharge planning and/or necessary referral to other members of the health care team.
- Participates in physician/team rounds
- Identifies unstable or complex patient care situations and facilitates appropriate intervention with appropriate resources.
- Recognizes emergency situations and initiates effective emergency care
- Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families
- Demonstrates the ability to utilize basic equipment in a safe manner.
- Facilitates patient discharge disposition or transfer by coordination of appropriate services
- Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings.
- Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn.
- Updates own knowledge base throughout the year.
- Completes all identified clinical competencies.
- Mentors nursing personnel in all levels of clinical practice.
- Communicates effectively with patients/families, providers interdisciplinary teams and ancillary services at all levels to assure coordinated efficient patient care.
- Documents the plan of care, interventions and evaluations in a clear and concise manner.
- Adheres to the established Performance Expectations for WVUM Employees in the areas of People, Service, Performance Improvement, and Shared Values & Culture
- Performs initial assessment and develops plan for new admissions in conjunction with the critical care attending physician and/or team.
- Ensures patient summary (i.e. Problem List) and orders are current, comprehensive and accurate.
- Ensures appropriate transition to next team when patient is ready to leave the ICU
- Performs appropriate procedures as directed by Critical Care Team and/or within scope of practice.
