CLINICAL CARE COORDINATOR (Ambulatory Care- Cardio- Obstetrics Clinic)

The University of Michigan Cardio-Obstetrics Clinic, housed within the Department of Obstetrics and Gynecology in the Von Voigtlander OBGYN Clinic and in close collaboration with Adult and Pediatric Cardiology, provides specialized, multidisciplinary care for pregnant and postpartum patients with pre-existing or pregnancy-associated cardiovascular disease. The clinic supports patients across the continuum of care—from preconception counseling through pregnancy, delivery planning, and postpartum cardiovascular follow-up.

The Cardio-Obstetrics care team includes obstetricians, maternal-fetal medicine specialists, cardiologists, advanced practice providers, registered nurses, medical assistants, social workers, dietitians, research staff, and a dedicated administrative team. The clinic operates within a team-based, patient-centered model emphasizing clinical excellence, coordination of complex care, health equity, and integration of research and education. This role is primarily clinical but also supports key research functions and collaborates closely with research staff and investigators.

Clinical Care & Coordination

  • Provide culturally responsive, age-specific nursing care to preconception, pregnant, and postpartum patients with cardiovascular conditions within a multidisciplinary Cardio-Obstetrics care model.
  • Utilize advanced clinical judgment and obstetric/ cardiovascular-specific nursing knowledge to assess, triage, and manage complex conditions.
  • Perform telephone, virtual, and in-person triage, including symptom assessment (e.g., chest pain, dyspnea, syncope, hypertension), referral coordination, and clinical decision-making per established protocols.
  • Support outpatient clinical operations through MiChart InBasket management, telephone triage, and coordination of urgent and routine follow-up.
  • Participate in care coordination activities including medication reconciliation, test and lab result management, order placement per protocol, obstetric intakes, postpartum follow-up calls, and patient education.
  • Assist providers during clinical visits and procedures as appropriate within scope of practice.

Patient Education & Support

  • Deliver patient- and family-centered education related to cardiovascular disease in pregnancy, including medication adherence, warning signs, lifestyle modifications, and postpartum cardiovascular risk.
  • Evaluate effectiveness of patient education and adapt teaching strategies to meet individual patient needs, health literacy levels, and cultural contexts.
  • Collaborate with social work and community resources to address social determinants of health and support continuity of care.
  • Develop and implement educational materials, standards, methods, and tools to improve patient care.

Interdisciplinary Collaboration

  • Work collaboratively with obstetrics, maternal-fetal medicine, cardiology, anesthesia, nursing, and ancillary services to ensure coordinated, high-quality care.
  • Problem solves with multidisciplinary teams across practice settings and the continuum of care both within and external to Michigan Medicine.
  • Serve as a clinical resource to nursing staff, medical assistants, clerical staff, and trainees regarding Cardio-Obstetrics workflows and patient care needs.
  • Provide functional supervision and guidance to assistive personnel as appropriate.

Documentation & Quality

  • Maintain accurate, timely, and comprehensive documentation in the electronic health record.
  • Prepare patient charts in advance of visits to facilitate efficient, high-quality care.
  • Identify unstable or complex clinical situations and initiate appropriate interventions, escalation, and referrals.
  • Demonstrate accountability for care delivered directly and care delegated to others.

Research activities

  • Participate in clinical conferences, Cardio-Obstetrics case reviews, and continuing education activities to maintain current knowledge of cardiovascular care in pregnancy.
  • Support clinical and translational research efforts related to Cardio-Obstetrics, as applicable.
  • Coordinate with research team to facilitate collection of data/ samples and scheduling of visits.
  • Perform phlebotomy, non-invasive specimen collections, intramuscular injections, and subcutaneous injections as a part of research protocols.
  • Maintain regulatory training, compliance with the IRB guidelines, and adherence to FDA regulations.
  • Participate in research subject clinic and home visits as necessary to support Cardio-Obstetrics research efforts.
  • Collaborate in educating nursing staff, students, residents, fellows, and other learners within the academic health system.

Other Duties

  • Perform other nursing duties as assigned to support clinic operations and patient care needs.

Required Qualifications

  • Current license to practice as a registered nurse in the State of Michigan
  • Required Educational Requirement; applicant must meet one of the following:
    1. Bachelors degree in nursing OR
    2. Associate degree or diploma in Nursing and a Masters degree or higher in Nursing
  • Minimum of three (3) years recent (within the last seven (7) years) experience in Labor and Delivery
  • Minimum of one (1) year of recent (within the last five (5) years) experience caring for cardio-obstetric patients within a maternal-fetal medicine ambulatory care clinic
  • BLS certification

Preferred Qualifications

  • Previous experience using clinical protocols to make decisions for adult patients and facilitating education for staff when new protocols are developed or modified.
  • Experience with MiChart, EPIC, Cisco Finesse, Word, and Excel
  • Certification in Ambulatory Nursing
  • Certification in Obstetric Nursing

Salary
Hourly range for RSAM Competent, Registered Nurses $43.10-$67.03/ hour

Evening Shift Differential-$3.00 / hour
Night Shift Differential- $4.00 / hour
Day Shift Weekend Differential- $2.90 / hour
Evening Shift Weekend Differential- $5.90 / hour
Night Shift Weekend Differential- $6.90 / hour
Charge Nurse Differential- $1.00 / hour

Hours/Week: 40 hours (1.0 FTE)

Shift/Hours/Days: Day shift (8-hour shift); Monday-Friday. Hours and days may be variable (nontraditional including further weekends/holidays commitment) based on patient and volume needs

Location: Von Voigtlander OBGYN Clinic and other work locations as needed to support patient populations seen in other locations.

This is a grant-funded position. Employment is dependent on continued funding and may be terminated if funding ceases.

Note: All new employees will be expected to float to a designated unit(s) in times of low census.

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
