Clinical Coordinator II/RN

Position Summary

Reporting directly to the Senior Nurse Manager, the Clinical Coordinator II (Registered Nurse II) assists physicians with patient care, health education and promotion, electronic medical record (EMR) management, coordination of patient care, and clinical protocols as needed. These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator II (Registered Nurse II) will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.

Responsibilities

  • Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
  • Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed.
  • Administers medications, injections, and immunizations in accordance with established protocols and observes, documents, and reports patient responses to treatment and medications.
  • Under limited supervision, executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.
  • Supports on-going clinical functions to provide compassionate patient-centered care.
  • Assists with daily staffing coordination and provides clinical coverage as needed, including coverage for same-day absences.
  • Handles telephone, My Chart and in-person triage (for symptom management) as well as patient education.
  • Assist with management of urgent and emergent patient situations in conjunction with care team leadership.
  • Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision.
  • Review and act on test results (lab, x-ray)
  • Review and manage communications from internal and external ?customers? inclusive of inbasket messages, updating immunizations, updating health maintenance items.
  • Performs venipuncture and point-of-care testing as required.
  • Maintains accurate laboratory and pathology specimen records and supports specimen tracking workflows.
  • Ensures infection control compliance within the practice.
  • Addresses measures to provide patient and family satisfaction.
  • May float to other clinics as needed.
  • Supports onboarding and training of new clinical staff; serves as a clinical resource and mentor of Certified Medical Assistants (CMAs).
  • Supports ongoing staff education of new medical procedures and techniques and ensures maintenance of clinical competencies.
  • Assists with monitoring and ordering clinical supplies, medications, equipment, and instruments.
  • Performs monthly maintenance and auditing of emergency equipment.
  • Participates in quality improvement activities and supports clinical best practices.
  • Provide chaperone duties as required by the Department?s Chaperone Policy.
  • Increases transparency, encourages event reporting, and advances the safety culture by participating in and facilitating safety huddles, patient safety activities, culture reinforcement, and shared learning. Tracks, documents, and reports risk management incidents, medication errors, and patient safety events.
  • Exercise skill in prioritizing assignments in order to complete work in a timely manner especially when there are changes in assignments, pressures of deadlines, competitive requirements and/or a heavy workload.
  • Participates in safety huddles, debriefings, and shared learning activities.
  • Perform other duties as assigned appropriate to scope, education, and/or training.

Minimum Qualifications

  • Bachelor?s degree or equivalent in education and experience.
  • Minimum of 2-4 years of relevant nursing experience; Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
  • Current New York State Registered Nurse License.
  • BCLS certification.

Preferred Qualifications

  • Outpatient OBGYN clinical experience.
  • Prior experience precepting or mentoring staff.
  • EMR experience (Epic preferred).
  • ACLS certification.
