Position Highlights

A Community Health Worker (CHW) serves as a vital link between healthcare providers and the community, particularly underserved populations providing outreach, education, and support to improve health outcomes. They help individuals and families navigate the healthcare system, access resources, and adopt healthy behaviors. CHWs often come from the communities they serve, enabling them to build trust and provide culturally relevant support. The CHW will be responsible for a number of different tasks that develop, deliver, and administer educational and community programs for external audiences through the Primary Prevention Mobile Health Unit. The Mobile Health Unit promotes and fosters lifelong learning and outreach activities for individuals, families, organizations, and communities. Job duties will include linkage to health and social services.

This job requires a blend of local and in-state travel with some overnight stays. Must be willing to work early mornings, late evenings, and weekends.

Duties & Responsibilities

Outreach and Engagement

  • CHW will conduct community visits, participate in community events, and build relationships to connect individuals with health and social services. They will conduct health screenings and provide referrals. They will drive and help maintain the Mobile Health Unit.

Health Education and Promotion

  • CHW will provide information on various health topics, including disease prevention, healthy lifestyles, and available resources by organizing support groups and educational workshops.

Navigating the Healthcare System

  • CHW will assist individuals in understanding and processing healthcare services, scheduling appointments, and following up via text, email, or phone call. They may also assist with applications for health insurance and other social services.
  • CHW will provide ongoing support to individuals with chronic conditions helping them manage their health and access necessary care. Playing a crucial role in improving health equity by bridging the gap between healthcare systems and communities, empowering individuals and families to lead healthier lives.
  • CHW will collect data on community health needs and relay this information to Mobile Health Unit Tucson Project Coordinator to inform program planning and service delivery.

  • Ability to communicate effectively with different groups/individuals.
  • Ability to handle confidential and sensitive information.
  • Skills in public speaking and presenting information to small or large groups.
  • Ability to work with in a team.
  • Knowledge of local healthcare systems and available social services.
  • Strong verbal and writing skills and ability to effectively communicate with diverse populations.
  • Strong organization skills and able to manage multiple tasks and responsibilities.
  • Ability to be empathetic and compassionate, able to build trust and rapport with community members.
  • Ability to identify and address barriers to healthcare access and other community needs.
  • Ability to understand and respect the cultural beliefs and practices of the communities they serve.

Minimum Qualifications

  • Bachelor's degree or equivalent advanced learning attained through experience required.
  • 1 year of relevant work experience required.

Preferred Qualifications

  • Past experience in community engagement and outreach required.
  • Fluent in the Spanish Language.
