Develop provider toolkits, nursing protocols, patient education materials, quick-reference guides, and other resources to support evidence-based maternal cardiometabolic care.

Serve as the primary clinical contact for participating healthcare practices and support implementation and sustainability of evidence-based practices.

Provide practice facilitation by assessing needs, identifying barriers, coordinating education and technical assistance, and helping practices integrate maternal cardiometabolic care into existing clinical workflows.

Facilitate communication and collaboration between participating practices, Maternal-Fetal Medicine (MFM) specialists, and the MHAP Hub team.

Coordinate telehealth protocols and workflows to support timely and effective consultation with MFM physicians.

Support the availability and appropriate exchange of remote patient monitoring information, including blood pressure and continuous glucose monitoring data, and assist practices with resolving workflow barriers.

Assist participating practices with patient navigation, including high-risk referrals, telehealth scheduling, resource identification, care coordination, and connection to healthcare and community resources.

Support quality-improvement activities related to hypertension bundle compliance, postpartum blood pressure follow-up, diabetes screening, telehealth utilization, referral completion, remote patient monitoring enrollment, and maternal outcomes.

Track educational activities, participation, and other program metrics and contribute to grant reporting and annual program reports.

Use program and quality-improvement data to identify opportunities to strengthen maternal cardiometabolic care and improve program outcomes.

Collaborate with physicians, nurses, program staff, healthcare organizations, and community partners to support implementation of MHAP services across participating healthcare systems and practices.

Perform other duties as assigned

• Please note that this position requires some after-hours and weekend shifts to provide full coverage for our resource access hotline. Hours may shift on a weekly basis.

Minimum Qualifications

Three (3) years of clinical experience, which provides knowledge, and understanding of medical terminology, patient diagnoses, treatment protocols and disease process.
One of the following:
Master of Social Work degree, or Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensure by the Missouri State Committee for Social Workers board, or
Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days, or
Occupational Therapist (OT) licensure by the Missouri Board of Occupational Therapy, or
Speech Language Pathologist (SLP) licensure by the Missouri Commission for Speech-Language Pathologists, and Certificate of Clinical Competency (CCC) certification from American Speech-Language-Hearing Association (ASHA), or
Physical Therapist (PT) licensure by the Missouri Board of Registration for the Healing Arts or PT Compact multi-state privilege. When primary state of residency changes, compact state PTs must apply under new state of residency within thirty (30) days.

Preferred Qualifications

Credentials: Current Missouri licensure as registered as a nurse.

• Education or professional experience in cardiometabolic care.
