Job Duties
- Foster continuity of care and facilitate transitions across inpatient and outpatient settings.
- Provide patient evaluation, assessment, and implementation of planned interventions via in person care and tele-management.
- Provide patient care to heart failure patients through infusion therapy with IV diuresis, electrolyte management, and other urgent treatment needs.
- Participate in the tele-management of heart failure patients, including symptom assessment, education and interventions as directed by providers
- Utilize various resources available within the Ambulatory Care to determine and meet the needs of the patient/family.
- Foster continuity of care and facilitate transitions across inpatient and outpatient settings.
- Other Duties as assigned.
Expectations
- Collaborate with Nurse Manager and other health team members on units providing care to patients to identify and support educational needs.
- Serve as a consultant to nursing personnel and other health care professionals caring for Heart Failure patients
- Participate in the development, modification and interpretation of policies and procedures, as needed.
- Participate in area nursing staff meetings as well as other departmental meetings.
- Facilitate timely communications to interdisciplinary staff regarding patient needs/plan of care.
- Monitor nursing care provided and utilize results of quality monitoring to initiate changes in practices, as indicated.
- Participate in continuous quality improvement by supporting changes recommended through the continuous quality improvement process.
- Assume responsibility for on-going professional development, attending appropriate continuing education, staff development programs, conferences and grand rounds as indicated.
- Maintain current knowledge of nursing and professional literature in regard to cardiology and heart failure.
- Participate with nurse manager in annual evaluation of own performance.
Required Qualifications
- Current licensure to practice nursing in the State of Michigan
- Five years of nursing experience as an RN
- 2 years experience as an RN within the last 5 years working with the Adult Heart Failure Patient population.

Preferred Qualifications
- 2 years Experience with tele-management of Adult patients as a RN in the ambulatory setting.
- Experience starting peripheral IVs within the last 2 years
- Experience with IV diuretics and electrolyte management as a RN
- Demonstrated Quality Improvement involvement.
Salary & Nursing Framework Level
This UMPNC RN posting is posted as Nursing Framework LEVEL C. Actual Nursing Framework LEVEL and salary will be determined at time of hire. Nursing Framework levels range from Level A to Level F.
Required qualifications must be met by the candidate in order to be interviewed and considered for the position. Posting may be filled after the initial 5-day posting period.
Schedule
- Hours/Week: (1.0 FTE) 80 hours bi-weekly (8 and 10 hours shifts available)
- Shift/Hours/Days: Day shift (8 or 10-hour shifts available); Monday-Friday. Hours and days may be variable (nontraditional including further weekends/holidays commitment) based on patient and volume needs
- Location: Cardiovascular Center and other offsites as needed including Northville health center, Brighton Center for Specialty Care, Briarwood 05 (Other work locations as needed to support patient populations seen in other locations)
