Job Summary

Provides direct clinical social work services to assigned resident case load of residents and families; Performs as an integral member of Interdisciplinary care team; Provides psychosocial data, assessments and goals through comprehensive assessments and follow up; Facilitates support groups and therapeutic sessions with patients and families; Assist in program development and presentation of educational in-services to staff and family members; Assists in field instruction to Social Work students as instructed by Director of Social Work.

Responsibilities

The duties include, but are not limited to:
SOCIAL WORK SERVICES: Provides direct clinical social work services to assigned case load in case management format.
ORIENTATION & TRAINING: Assists in orientation and training of staff as instructed by the Director of Social Work.
DOCUMENTATION: Provides required documentation... Coordinates benefit entitlements for current residents...
COVERAGE: Provides occasional social work coverage...
CARE TEAM: Performs as an integral member of interdisciplinary patient care team.
SUPPORT GROUPS & SESSIONS: Facilitates support groups and therapeutic sessions with Patient/residents and families.
PROGRAM DEVELOPMENT & PRESENTATION: Assists in program development... Facilitates resident participation...
INSTRUCTION: Assists in field instruction to Social Work students...
OTHER: Perform other duties as assigned.

Required Qualifications

Master's degree from an accredited college or university in Social Work from a program accredited by the Council on Social Work Education (CSWE) and two years of prior social work experience which may include clinical graduate internship experiences. Licensed (LCSW) or license eligible MSW in the state of Georgia. License must be obtained prior to end of six-month probationary period.

Preferred Qualifications

Experience in health care and long-term care setting, gerontology, and end of life care is strongly preferred.
Experience with group work and program development is highly desirable.

Certifications/License

Shift/Salary/Benefits

Shift: Days/M-F (Work outside of normal business hours may be required)
Pay Band: B12
Salary to be commensurate with the qualifications of the selected candidate within the established range (generally minimum midpoint) of the position.
