The Cognitive Care Network Community Navigator is responsible for developing relationships with health care providers (primarily MD, DO, APRN) in designated Kansas counties and, in collaboration with the Director, advance the use of clinically indicated cognitive screening tools, enlist providers in the Cognitive Care Network, and advance research referrals.
