Substance Use Nurse II
Job Description Summary
The Substance Use Nurse II is an RN who practices as an integral member of an interdisciplinary, collaborative care team focused on providing accessible and site-based primary care services to individuals experiencing homelessness residing in recovery residences. The care team will be focused on expanding low-barrier buprenorphine treatment access and warm handoffs to longitudinal care. On-site clinical services will include RN-led wound care, complex care management, and harm reduction education, among other general primary care needs.
Job Responsibilities
- Provide patient-centered, trauma-informed care, for individuals experiencing homelessness residing in recovery residences.
- Lead wound care (xylazine wound experience preferred), complex care management, harm reduction education, and general primary care needs.
- Document patient encounters in data collection system and electronic health record.
- Work collaboratively with an interdisciplinary care team including physicians, care managers, substance use navigators and certified recovery specialists, etc.
- Other duties and responsibilities as assigned
Qualifications
- Bachelor's degree and 5-7 years of experience along with Registered Nurse with Licensure in Commonwealth of Pennsylvania without restriction or limitation required. BSN preferred.
- Bilingual (Spanish) preferred.
