If volume were a personality trait, I would have listed it on job applications. My upstairs neighbours could name every track on my Friday-night cleaning playlist without leaving their kitchen, and I considered that a feature rather than a bug.
Music is one of the few things that behaves like medicine without a prescription or an appointment, and with no co-pay. It changes how we feel, how we move, how much pain we notice, how well we sleep, and how clearly we think. In the wrong dose, it also damages the very hardware it's supposed to soothe.
The evidence splits into five solid benefits and five serious risks. Nobody tells you that both lists come from the same playlist. The dividing line is mostly two things: how loud, and how long.
I learned this after a weekend of over-ear headphones at what I called "work volume." By Monday I was asking colleagues to repeat themselves. My left ear had the high-frequency range of a bedside radio left on all night. That is the least glamorous finding in music research: your cochlea does not negotiate.
Five benefits research keeps confirming
The benefits below appear across randomised trials and systematic reviews, not just in the comments section of a lo-fi playlist. They hold up best when people choose music they actually like.
- Stress reduction is the most consistent finding. A meta-analysis published in Health Psychology Review pooled dozens of randomised trials and found that music interventions reliably lowered self-reported stress as well as physiological markers such as heart rate and cortisol. The effect held in medical and workplace settings, along with laboratory stress tests, which undercuts the idea that the benefits come mainly from sitting somewhere comfortable.
- Music works as a mild painkiller. Cochrane reviews going back to 2006 show that listening to music around surgery reduces pain intensity and cuts the amount of opioid medication patients request. A related Cochrane review of people with cancer found similar reductions in anxiety and pain. The effect sizes are modest rather than miraculous, but patients consistently report feeling more in charge of the experience, which may be part of the mechanism.
- Exercise feels easier at the right tempo. Most of this research comes from sports science, where studies have repeatedly shown that synchronous music — the kind you step or pedal to — improves endurance and lowers perceived effort. The sweet spot sits around 120 to 140 beats per minute, roughly the speed of an upbeat pop chorus or a determined power-walk.
- Sleep improves when the tempo slows down. A Cochrane review of adults with insomnia found that listening to music for 25 to 60 minutes before bed helped people fall asleep faster and report better sleep quality, although the evidence was rated moderate rather than dazzling. The consistent pattern across trials: a tempo below about 100 beats per minute, which rules out most of my Friday-night playlist.
- Cognitive performance gets a modest, conditional boost. The Mozart effect — the idea that a little classical music makes you smarter — was overplayed from the start. The original effect was small and short-lived, confined to one spatial-reasoning task. What survives scrutiny is narrower and less romantic: background music with a moderate tempo and no lyrics can help on repetitive tasks, while lyrics tend to disrupt anything that involves words.
Five risks that deserve the same volume knob
The risks are less glamorous than the benefits, mostly because they're chronic rather than cinematic. You won't feel a hair cell die. That is the problem.
- Noise-induced hearing loss is the headline risk. The World Health Organization's Make Listening Safe programme estimates that more than a billion teenagers and young adults are at risk from unsafe listening on personal audio devices and in loud entertainment venues. Hair cells in the inner ear convert sound into signals, and once they're damaged they don't grow back. The WHO's guidance pairs volume with time: roughly 85 decibels for up to eight hours, with less time allowed as the level climbs.
- Loud listening can leave a permanent ringing. Tinnitus — the perception of sound when no external sound exists — is one of the most common consequences of noise damage. For some people it's a faint hiss in a quiet room; for others it's intrusive enough to disrupt sleep and concentration. The damage that sets it off is usually permanent, which makes prevention the only treatment that reliably works.
- Headphones in traffic have been lethal. A study published in Injury Prevention reviewed United States incident reports from 2004 to 2011 and identified more than a hundred cases in which pedestrians wearing headphones were killed or injured, most involving trains. The researchers pointed out that headphones create two problems at once: distraction, and the loss of warning sounds that would normally override a lapse in attention. The count also rose over the study period.
- Fast, loud music changes how people drive. Experimental research, including work on young-novice drivers from Israel's Ben-Gurion University, has linked fast-tempo background music to faster driving, more frequent lane changes, slower responses in complex traffic, and riskier overtaking. The effect strengthens above roughly 120 beats per minute — the same tempo that makes a gym session fly by.
- Sad music can deepen a low mood in the wrong state of mind. Research on music and rumination suggests that people with depression sometimes select sad music because it matches how they feel, and the match can keep the mood spiral going rather than release it. Other studies find the opposite for many listeners, who describe sad music as comforting. The contradiction is the point: individual state matters more than genre.
Where the evidence gets wobbly
Almost every benefit on the first list comes with a methodological asterisk. You can't blind a participant to music the way you would blind them to a pill: the person in the silence condition knows they got silence. That knowledge can inflate effects, especially for subjective outcomes such as mood and sleep quality.
Study quality is uneven for the same reason. Sample sizes are often small, and interventions range from "choose your own playlist" to "listen to this exact 13-minute ambient track." Many trials also rely on self-report. The Cochrane reviews are explicit about this — their insomnia review describes the evidence as moderate at best, even though the results point in a useful direction. Music's benefits are real enough to act on, but not precise enough to prescribe like a dose of metformin.
Individual taste complicates things further. The same song that calms one person can irritate another, and most trials don't fully account for preference. That's why blanket advice to "listen to classical music while working" fails: the genre matters less than whether the music is familiar, predictable, and not competing for your verbal bandwidth.
A few rules that survive contact with your actual playlist
The practical advice from hearing-health researchers is boring, which is another way of saying it's cheap and effective.
- Keep the volume under about 60 percent on most phones, and if you can't hear someone speaking at arm's length, turn it down.
- Take quiet breaks between long listening sessions.
- Use noise-cancelling headphones in loud environments so you're not fighting background noise with more volume.
- Leave the earbuds out near traffic and at railway crossings, where warning sounds do most of the work.
- If you need audio to fall asleep, use a low-volume speaker placed away from the bed rather than earbuds in the ear canal all night.
- At concerts and clubs, consider musicians' earplugs, which reduce volume more evenly than foam plugs and still let you hear the music.
I still clean to the same Friday-night playlist; I just do it with the volume set so the neighbours have to make their own soundtrack. My left ear and I have reached an agreement: I supply the music, it supplies a slightly narrower range of high frequencies, and neither of us pretends that trade was free.
