Job Summary
Counseling Center welcomes applications for a year-round, full-time Care Coordinator for students in need of longer term or more specialized treatment; coaching and skills based training for students navigating care, campus and community referral follow-up, collaboration and outreach with campus partners who provide case management for Drexel students, and active collaborations with community mental health providers and agencies, including those who offer free and low cost treatment. This person will also provide consistent follow up to ensure that clients’ needs are being appropriately met off campus.
Essential Functions
- Provides case management for students requesting or are recommended for a transfer of care out of DUCC services.
- Serve as a bridge to appropriate community resources (whether they completed the referral, quality of care from various providers, etc.); facilitate access and communication; monitor and ensure appropriate services are delivered.
- Assists with coordination of hospitalization and discharge planning and advocating for students as appropriate in helping them access care.
- Assists in tracking students referred to the community and communicates with them as needed about continuity of care.
- Consult with parents, faculty and staff when appropriate to ensure quality care to students.
- Provides ‘bridge’ services as needed.
- Develop and maintain comprehensive knowledge with community providers and agencies to expand and maintain mental health referral networks and relationships.
- Assists in improving internal and external referral processes.
- Complies, maintains, and updates several types of referral resource information, including maintaining and updating database.
- Provide follow up contact for clients of Counseling & Health who are engaged in treatment with community medical providers.
- Maintains timely records regarding client status, in keeping with Counseling Center record keeping policies.
Required Qualifications
- Minimum of a Bachelor's Degree in Social Work, Psychology or related field or the equivalent combination of education and work experience. (Please review the Equivalency Chart for additional information.)
- Minimum of 3 years of experience in mental health counseling, case management, care coordination, preferably in a higher education setting.
- Excellent written, verbal and time management skills.
- Ability to work independently as well as establish relationships, and function within a team.
- Experience working with multicultural and marginalized identities.
- Ability to effectively manage students with significant mental health needs.
- Strong interpersonal skills.
Preferred Qualifications
- Master in Social Work with 1 year of experience mental health counseling, case management, care coordination, preferably in a higher education setting.

Additional Information
This position is classified as Exempt, grade J. Compensation for this grade ranges from $47,490 - $71,230 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
