Clinic Nurse - Healthcare for the Homeless Suitcase Clinic/IIHHS (Part-Time)
General Information:
The Healthcare for the Homeless Suitcase Clinic, a healthcare initiative dedicated to improving the health of the local homeless population, is accepting applications for a part-time Clinic Nurse. The Clinic Nurse collaborates with other professionals to evaluate patients' physical and mental health conditions as well as other client needs. The Nurse serves as an advocate who helps patients understand their health status and how to improve it as well as a liaison between patients and clinicians, staff, and community organizations to ensure that the desired client outcomes are achieved. The Clinic Nurse reports to the Program Director for the Healthcare for the Homeless Suitcase Clinic.
Duties and Responsibilities:
- Attend all clinics at shelter sites and assist in managing clinic setup and activities
- Conduct physical and mental health assessments with homeless clients to determine needs, including health care services, emotional support, and other practical needs
- Work with individual homeless clients to develop a service plan based on an assessment of strengths and challenge areas
- Encourage clients to establish a medical home with a primary care provider and initiate referrals to medical, mental health, substance abuse, and/or dental professionals
- Work closely with the Care Coordinator to assist clients in identifying and accessing available resources including medications, financial resources, public benefit programs, and services from other organizations
- Collaborate with other Suitcase Clinic staff and volunteers and community agencies to coordinate services and avoid duplication of efforts
- Maintain accurate and up-to-date records of client contacts, clinical notes, services needed, and services provided within appropriate deadlines
- Participate in weekly staff meetings and report on patient needs and clinic activities
Qualifications:
- RN with extensive experience
- Nursing licensure in the state of Virginia
- Knowledge of trauma, substance abuse, mental health and medical conditions common to people experiencing homelessness
- Excellent interpersonal skills and experience working with people experiencing homelessness
- Excellent oral and written communication skills
- Strong computer skills (including Microsoft Word, Access, and Excel) and the Internet
- Ability to work effectively as part of a team with volunteers
- Ability to plan, organize, and prioritize multiple tasks and accomplish goals in a timely manner
- Ability to work under stressful and difficult emotional situations
- Experience working with community resources
- Must be available to travel, and to work variable hours (ranging from 8am-9pm) as determined by clinic schedule
- This part-time wage position will work up to 25 hours per week, with hours fluctuating based on need. This position will work no more than 1,300 total hours per year and is not eligible for benefits.
