Clinical Behavioral Health Care Specialist
Position Summary
Serves as a licensed behavioral health clinician who works with a clinical team that advances behavioral health integration in the primary care setting using the principles of the Collaborative Care Model.
Job Responsibilities
- Provides evidence-based behavioral health interventions and psychoeducation.
- Assesses and systematically collects and tracks data in a patient registry related to patient behavioral health status and works with the care team to develop the plan of care.
- Participates in-person or virtual huddles when shared patients are discussed.
- Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
- Develops and maintains a working knowledge of community resources and managed care review processes.
- Assumes responsibility and accountability for effectively managing behavioral health care of individual patients.
- Documents history, assessments and interventions to reflect comprehensive and integrated approach to patient care. Ensures complete, accurate and timely documentation in the electronic medical record.
- Identifies learning needs of the patient and families, provides patient education; educates by adapting to patient and family learning style utilizing the teach-back method.
- Attends team meetings and case conferences to present and discuss patient disposition and outcomes data.
- Assists with the coordination of care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools). Assists with management of urgent and emergent patient care situations.
- Assists with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness.
- Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
- Performs other related duties as assigned and as appropriate to scope, education, and/or training.
Education
Master's Degree
Experience
Licensed Clinical Social Worker (LCSW)/Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC). Experience working with individuals and families affected by mental illness. Experience providing evidence-based psychotherapeutic approaches. Ability to thrive within a complex medical system. Working understanding of the healthcare system.

- Ability to serve as a mentor and resource to others.
Licenses and Certifications
Licensure in social work (LCSW or LMSW) or mental health counseling (LMHC).
Working Conditions/Physical Demands
Ability to work from home or on-site.
