The care provided addresses each of the 4 Geriatric “M's,” guiding our participation in the Age Friendly Health System initiative. The division currently has thriving team-based care programs in the areas of Geriatric care, including its Patient-Centered Medical Home; dementia and other cognitive disorders in its Aging Brain Care program; and mobility in its Faint and Fall clinic.
