Clinical Nurse Adult Congenital Heart Disease
Job Summary
The Division of Pediatric Cardiology in the University of Utah School of Medicine is seeking a Clinical Nurse to support the Adult Congenital Heart Disease (ACHD) program. This role assists in the coordination and management of clinical services for patients with congenital heart disease across the lifespan, including adolescents and adults. The Clinical Nurse will work closely with cardiologists, advanced practice providers, and multidisciplinary care teams to support patient care coordination, clinical follow-up, and communication with patients and families. The Pediatric Cardiology Division is based at Primary Children's Hospital in Salt Lake City, Utah.
Responsibilities
Adult Congenital Heart Disease Clinic Coordination
- Respond to patient questions and concerns received through phone messages or electronic medical record messaging.
- Coordinate patient care activities including follow-up appointments, diagnostic testing, and specialty referrals.
- Review and follow up on laboratory results and diagnostic studies completed during clinic visits.
- Assist with prescription management, medication renewals, and patient education regarding medication use.
- Support anticoagulation monitoring and coordination for patients requiring specialized management.
- Facilitate referrals to subspecialists such as maternal fetal medicine, vascular medicine, gynecology, and other relevant services.
- Provide follow-up communication with patients after emergency department visits, hospitalizations, or missed clinic appointments.
- Maintain and update ACHD patient care documentation including health passports and other patient care records.
- Complete administrative documentation such as disability forms, school or employment paperwork, and driver's license medical clearances as needed.
Pre-Clinic Coordination
- Coordinate pre-visit diagnostic testing and imaging as directed by cardiologists or advanced practice providers.
- Schedule and coordinate clinic visits for new and established ACHD patients.
- Assist with coordination of complex visits for patients traveling from outside the region who require multiple tests or consultations.
Clinic Day Responsibilities
- Support providers during ACHD clinic sessions by assisting with care coordination and follow-up planning.
- Arrange recommended follow-up services such as nutrition consultation, sleep studies, imaging studies, or genetic testing.
- Track and maintain databases of pending testing results including genetic testing and other specialized studies.
- Coordinate with insurance services, social work, and ancillary departments to support patient care needs.
Post-Clinic Follow-Up
- Coordinate referrals and follow-up care recommended during clinic visits.
- Maintain clinical tracking systems and patient care databases for the ACHD program.
- Ensure follow-up visits are scheduled and required documentation is completed in the electronic medical record.
- Assist with requests for outside medical records and coordination of procedural orders.
Program Support
- Support quality improvement and clinical program initiatives within the ACHD service.
- Participate in team meetings and collaborate with providers and staff to improve care delivery for ACHD patients.
- Perform other duties as assigned to support the needs of the Adult Congenital Heart Disease program and Pediatric Cardiology Division.
