"Clinical Nurse II -Clinic - Internal Medicine- PT Days"

Position Summary: Incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. Assists with procedures, patient education, outpatient and post hospital discharge follow-up including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation.

Required Qualifications:

• Previous experience in urgent care, emergency room or critical care setting
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Minimum two years demonstrated nursing experience in an ambulatory care setting
• Minimum of six months demonstrated nursing experience
• Demonstrated ability to triage patients and assign level of care
• Current California RN license
• All external candidates must have a Bachelor of Science in Nursing
• Ability to work variable shifts including evenings, nights, weekends, and holidays
• Ability to read, write and communicate effectively in English
• Ability to maintain flexibility and work well in a constantly changing environment
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System

Preferred Qualifications:

• Understanding of applicable regulatory requirements
• Pediatric ambulatory experience
• Bilingual skills to communicate effectively with patients and families
