Position Summary/Department Summary

This 20-hour position covers the Bridges Adult Transition Program's clinical social work needs, including inpatient consultation and virtual visit support for young adult patients and their families (with some limited inpatient Cystic Fibrosis coverage). Social work is an essential part of the interdisciplinary care for a population of young adult patients transitioning to adult care programming. This population has specialized needs and is at particularly high risk for several complex social and medical needs where social work is consultative.

Key Responsibilities

• Psychosocial assessment, crisis intervention, coordination of care, linkages to community services, and consultation regarding protective concerns, especially related to disabled adults and interpersonal violence. Psychoeducation to be provided on multiple issues including navigating complex systems, reproductive health, management of family issues and termination of long-standing relationships with pediatric providers.
• A strong multidisciplinary team approach and shared clinical responsibility with team members, in both inpatient and virtual visit settings.
• Opportunities for engagement in education, programmatic and QI efforts to improve systems of care.

Minimum Qualifications

Education

• Master's degree in social work.

Experience

• Social work experience in medical environment, bereavement, and disability issues helpful.
• Flexibility and ability to work in a fast-paced dynamic environment.
• Excellent oral, written, and interpersonal communication skills.
• People of diversity strongly encouraged to apply.

Licensure/Certifications

• LCSW in the state of MA, LICSW in the state of MA preferred
