Community Case Manager or Coordinator

Job Summary/Basic Function

We are looking for either a Case Manager or Case Coordinator and the final offer will be made depending on the candidates experience and qualifications.

Case Manager: To provide case management services and prevention education in the areas of substance abuse, HIV/AIDS, STIs, and hepatitis C prevention to adults in the community who are homeless or marginally housed and in need of treatment services.

Case Coordinator: To provide case coordination and ensure that program participants are receiving the care that they need in the areas of substance abuse prevention and treatment, HIV testing, and HIV prevention services (including referrals and follow up if additional professional services are needed) as well as providing behavioral health educational interventions to all participants.

Minimum Qualifications

Case Manager

  • Bachelor's degree in education or social/behavioral science or other science field.
  • At least six months experience (paid, internship, or volunteer) in a community setting providing any type of case management, educational, or support services.
  • A valid clinical license or certification may replace the degree if the applicant has at least three years' experience.

Case Coordinator

  • Bachelor's degree in a social or behavioral science field such as public health, education, social work and a minimum of 5 years' experience (which can include an unpaid and/or volunteer work for up to one year of experience) providing some type of direct services. OR an Associate's degree and a minimum of seven years' experience (which can include an unpaid and/or volunteer work for up to one year of experience) providing some type of direct services.
  • Direct services experience can include case management, tutoring or teaching, providing support services and linkage to care, etc.
  • Other experience involving working directly with participants in a program may also be considered.

Preferred Qualifications

Case Manager: Knowledge of HIV risk and substance abuse.

Case Coordinator: Licensure or other professional credential in the area of counseling, social work, prevention, public health, and/or education. Experience in behavioral health as well as experience in HIV related work.

Essential Functions and percent of time

Case Manager

  • Provide case management services to enrolled participants. 50%
  • Conduct small and large group education sessions. 10%
  • Conduct HIV and hepatitis C rapid testing and related education related to each. 10%
  • Recruit participants for the grant program. 10%
  • Conduct intake and follow-up interviews with recruited participants and document all activities and information collected. 20%

Case Coordinator

  • Provide case coordination between agencies, including our program, participating partners, and referral locations in the community. 45%
  • Provide additional case management and support to participants and document all activities. 20%
  • Conduct prevention education in both individual and group settings on topics including substance abuse, mental well-being, HIV and other STIs, and education related to testing for and preventing of STIs. 20%
  • Recruit participants and conduct intake and follow-up interviews. 15%

Working Conditions

Local travel required. Length of appointment is one year, annually renewable, contingent on grant funding. Special Contract terms and conditions of employment apply.
