"Community Health Worker Intern"

Salary: $15.14 to $17.04

Description: Observes program efforts by UH Community Health Workers (CHW) who provide outreach, navigation and eligibility assistance for participants of various programs.

1. Serves as point of contact between community/neighborhood and the UH CHW initiative.
2. Builds rapport with both CHW's as well as community members and families participating in the UH CHW initiatives.
3. Assists CHW's in recruiting program participants for study.
4. Observes CHW's home/site visits that identify needs for services and provides information and referrals to diverse resources.
5. Provides input to CHW's on case record documents as needed.
6. Partners with CHW's and staff members to assist in contacting community members participating in UH initiatives.
7. Adheres to policies and procedures of the organization and CHW Initiative programs.
8. Maintains partnerships with community-based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHW's community/neighborhood.
9. Performs other job-related duties as required.

MQ: Education: Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED. Experience: No experience required.

Additional requirements:

• Spanish speaking preferred/required
• Must have an active CHW certification

This position is grant-funded and will have an end date of August 31, 2027.
