Habilitation Counselor

Rutgers, The State University of New Jersey, is seeking a Habilitation Counselor for the Edison Outpatient Treatment within Rutgers University Behavioral Health Care.

The purpose of the Habilitation Counselor position is to provide site and community-based case management and supported employment services to individuals receiving treatment services through UBHC's Outpatient Dementia Clinic.

Among the key duties of this position are the following:

  • Supports individuals with Dementia and their family/caregiver to gain access to needed medical, social, education, mental health, addiction, co-occurring, and other services.
  • Demonstrates the ability to engage and establish a working relationship with and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders who present engagement challenges.
  • Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for individuals served and family members.
  • Maintains appropriate clinical records according to the time frames and quality guidelines established by UBHC policy and applicable accrediting organization standards.
  • Maintains knowledge of trends and developments in the field.
  • Contributes to and supports the mission, vision, values, goals and objectives of UBHC.

Minimum Education and Experience:

Bachelor's Degree in the mental health field or other appropriate human services related field plus one (1) year of relevant mental health experience.

Physical Demands and Work Environment:

  • Ability to speak clearly and write proficiently.
  • Hearing is essential.
  • The ability to focus on the task at hand despite numerous interruptions.
  • Must be able to sit for extended periods of time and document into an electronic medical record.
  • The employee spends much time driving, walking, standing, bending and talking with consumers in the community located throughout the County.
  • Transport of consumers in state vehicles.
  • The employee can expect to lift up to 30 pounds.
  • The work environment includes both office based services and community outreach.
  • Individual wellness checks and crisis intervention required.
  • Subject to unpredictable situations, patient crises, to include behavioral, medical and psychiatric.
  • Occasionally exposed to patients exhibiting assaultive behaviors.
  • Exposure to blood borne pathogens that requires use of personal protective equipment.

Special Conditions:

May work at least one (1) evening per week and one (1) weekend day per week.
