Position Overview:
To assist clients living in the community to link to necessary medical, behavioral health, housing, social, employment, educational, and other services essential to meeting basic life needs.
Sign-on Bonus: $3000 paid out over the first year of satisfactory employment.

Requirements
- <p>Qualifications:</p><ul><li>BA/BS in a human services field with a<strong> minimum of one-year’s experience</strong> in a behavioral health setting is required OR AA/AS in a human service field with a <strong>minimum of two-years experienc</strong>e in behavioal health setting. </li><li>Must have understanding/knowledge of the behavioral characteristics and psychosocial needs of the target population.</li><li>Strong written and verbal communication skills are necessary to maintain documentation and work effectively with clients, providers, and the healthcare community.</li><li>Skills & Knowledge Behavioral health</li><li>Bi-lingual English and Spanish speaking are a plus.</li><li>Must have own car, a valid driver’s license, and a driving record that is satisfactory to our insurance carrier.</li><li>Criminal History Background Check and fingerprinting are required.</li></ul><p>HDSW is an Equal Opportunity Employer</p>
