Description: The UH Health Family Care Center is a Federally Qualified Health Center (FQHC) Look-Alike, committed to delivering comprehensive, integrated health services to underserved populations in Houston. The center serves residents of the Greater Third Ward, East End, South Houston, and surrounding neighborhoods, while welcoming patients from across the region. Family Care Center integrates primary care, behavioral health (including psychology and psychiatry), women’s health, pediatrics, and chronic disease management. The clinic partners with community organizations and public agencies to expand access, reduce health disparities, and support wellness in historically underserved areas. UH Health Family Care Center is positioned to scale its impact and serve as a vital anchor for community health in Houston. Provides senior level professional nursing care in an organized and effective manner at the Clinic. Screens patients and obtain medical information and administers medication as prescribed. Monitors proper maintenance and use of medical supplies and equipment within the Clinic.
- Assists support staff in continuous quality improvement, emergency situations and execution of physician orders.
- Monitors and orders inventories of medical supplies for clinic and laboratory; facilitates repair of instruments; order supplies for patient use.
- Administers medications and treatments as prescribed by physician; keeps Chief Nurse advised on pertinent patient information, problems, emergencies or crisis intervention.
- Provides general nursing care to patients, monitors vital signs, administers medication and reports patient's condition to physician.
- Triages patients and provides direct assistance to physicians and providers; assists with patient care (referrals, specialty care, diagnostics, care coordination, etc.)
- Assists with clinic policies and procedures, including clinical partner referral process and patterns.
- Cleans and maintains equipment and examination/treatment rooms; restocks supplies; maintains autoclaving of instruments and keeps Chief Nurse advised of supply deficits.
- Prepares or compiles charts for specialty clinics, obtains laboratory reports and submits reports to physicians.
- Collaborates with Nurse Manager and other clinic staff to investigate and resolve grievances and complaints from patients and staff.
- Serves as a clinical resource for workflow or patient triage questions.
- Mentors level 1 LVNs and new staff members and ensures adherence to clinical policies.
- Participates in performance/quality improvement and staff development initiatives.
- Maintains vaccine inventory and routine reporting
- Orders and monitors medical supplies, ensuring exam rooms are stocked.
- Ensures infection control standards and safety protocols are followed.
- Performs other job-related duties as assigned.
MQ: Education: Requires specialized training in basic trades, principles, procedures, practice, routines or techniques in a specific area or trade which might normally be acquired through up to 18 months of education or training beyond the high school level. Vocational competence in the operation of mechanical or electronic equipment may be required. Experience: Requires a minimum of three (3) years of job-related experience. - Department is willing to accept education in lieu of experience. - Must be licensed in Texas as an LVN or LPN.
