Licensed Practical Nurse II - Carbondale (2736)
The position performs nursing care procedures under the direct supervision of the RN, Charge Nurse, Nurse Administrator or M.D. /D.O. provider. The primary responsibility of the position is to ensure that safe, quality, and efficient health care delivery is provided to patients under the direction of a Medical Doctor (MD or DO), Physician Assistant (PA), or Registered Nurse (RN). This position will foster a team approach by working collaboratively with the patient, family, physician, and other members of the health care team to ensure coordination of services. Actively participates in provider and staff team projects (including daily huddles but not limited to) in order to enhance patient care coordination and patient satisfaction.
Clinical Duties - 70%
- Assists Physicians, Physician Assistants, Registered Nurse, and/or LPN lls with exams, minor procedures and surgeries.
- Prepares patients for physician exams by accurately taking and recording vital signs, history, medications, procedures, and instructions according to policies and unit standards.
- Assisting providers as a scribe when necessary.
- Performs venipuncture and assists with CLIA waived lab tests.
- Administers medications and immunizations according to departmental policies.
- Processes medication refills according to established policy.
- Performs screenings such as visual and yearly wellness recommendations.
- Schedules patients with acute and urgent problems per scheduling policy. ?
- Acts quickly and appropriately in response to change in patient condition per doctor's 0I·ders.
- Identifies patient signs or symptoms that indicate a significant variance and informs RN and/or physician in a timely manner.
- Provides and documents patient care within the standards of practice, policies, procedures and department protocols.
- Processes pre-certifications and makes referral appointments.
- Collects specimen samples from patients of all age groups using appropriate lab procedures and two patient identifiers. Takes full responsibility for specimen from collection to delivery.
- Investigates and follows-up on all patient identification and specimen collection discrepancies.
- Notifies supervisor and/or physician with appropriate questions or concerns.
Clinical Support - 10%
- Sets up procedure room.
- Cleans, sterilizes, and packages instruments and equipment for diagnostic procedures and treatment as required.
- Assists in inventory of clinical supplies including checking of expiration dates.
- Maintains clean, well-equipped exam rooms, laboratory, and procedure rooms.
- Differentiates among emergent, urgent and non-urgent patients.
- Observes safety precautions for patients and staff.
Patient Education - 10%
- Provides clear, accurate information and thorough instructions to patients under the supervision of RN and/or provider.
- Provides patient with wellness Information and education in their disease management processes,
- Provides verbal and written education materials to patients under the supervision of an RN and/or provider.
Administrative Duties - 5%
- Assists supervisor with report completion as directed
- Assists in quality assurance and improvement (Le. Repeat BP checks, nurse standing orders).
- Prioritizes work effectively and seeks guidance from supervisor as needed
- Screens calls from home health agencies, nursing homes and other health care providers
- Collects and documents patient data over the telephone within the clinical standards
Education - 5%
- Attends nursing in-service, continuing education programs, and department meetings
Minimum Qualifications:
Graduation from an accredited program for Licensed Practical Nurses.
Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
Preferred Qualifications:
Six months of nursing experience in a health care facility (such as a hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities).
Condition of Employment:
Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
