Patient Care
Utilizes the nursing process and established protocols for the individuals and families who are patients of the Pediatric Clinics. This includes direct patient care, telephone triage, and patient education.
- Facilitates smooth patient flow by maintaining the prescribed schedule, preparing the exam room for the primary provider, and obtaining any relevant lab and x-ray reports prior to the visit.
- Prepares for clinic by ensuring required information is available at time of appointment, including but not limited to: lab tests, past immunization history, new born screen, etc.
- Obtains the basic assessment data on patients prior to the patient visit with the provider. Skillfully obtains blood pressures on infants and children, utilizing a variety of methods. Recognize readings which are not constant with normal ranges. Accurately documents the information in the electronic health record.
- Performs diagnostic tests as ordered by the provider.
- Assists the provider with the exam and procedures according to department guidelines.
- Schedules diagnostic tests, as ordered by the provider and informs the patient/family of necessary data regarding the testing.
- Processes prescriptions and refills according to provider's orders.
- Administers medications or treatments to the patient, as ordered by the provider, according to department guidelines under direct authorization of the provider. Utilizes proper procedure for administering medication and treatment with special consideration for patient safety.
- Give injections according to the departmental guidelines written and approved by the clinic director after successful completion of a competency exam and preceptorship. Medication administration is at the direct authorization by a physician, either verbally or written.
- Assist with clinic lab logs recording daily labs. Processes labs in a timely manner for follow up by the physician. Informs patients of results as directed.
- Maintains well-stocked exam rooms.
- Collects soiled linens and supplies and processes appropriately.
- Assists in maintaining adequate inventory of supplies and vaccines.
- Assists in maintaining supply of patient education materials, which are routinely stocked in the clinic.
- Assists in the completion of referrals for all managed care patients.
- Proficiently uses the electronic health record systems in support of quality care, including timely documentation of all patient encounters.
Telephone Triage/Patient Education
- Screens telephone calls from patients and evaluates the need for the patient to be seen or provided with health care information.
- Provides health care information to patient/families for treatment of acute and/or episodic minor illness according to protocols.
- Provides diagnostic study results to patient and documents same.
- Documents information conveyed by the caller pertinent to the patient encounter for the medical record: Records date; time and caller information specific to each encounter. This includes the time the call was received and completed. Proficiently utilizes the electronic health record according to established protocol.
- Documents the patient's history information, vital signs, allergies, current medications, weight and chief complaint for the record. Documents all information provided to the caller including follow up instructions. Confirms spelling of patient and callers name and phone number for each encounter.
- Organizes the phone call that requires follow up. Checks out to a phone nurse when scheduled absences occur.
- Consults with the physician for direction as indicated.
- Processes prescriptions per provider orders and refills per department policy. Documents all information regarding prescriptions in the medical record.
- Schedules outpatient testing per provider's orders. Utilizes the appropriate site of service based on insurance policies. Submits referral requests within the electronic health record per protocols.
- Makes ER or urgent care referrals for patients that are required as a result of the encounter per standing department protocols.
MISCELLANEOUS
- Coordinates the patient's care with outside agencies by communicating information regarding referrals to all concerned individuals.
- Attends in services and conferences to expand knowledge.
- Participates in training and education of residents, medical students and new employees.
- Other duties as required.
Minimum Qualifications
- Graduation from an accredited program for Licensed Practical Nurses.
- Eligibility for licensure as a Licensed Practical Nurse by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
Preferred Qualifications
6 months of nursing experience in a health care facility (such as a hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities)
