Job Summary:
The med-peds physician will provide comprehensive care to both adult and pediatric patients across a wide spectrum of health conditions, ensuring continuity of care from infancy through adulthood. The ideal candidate will be well-versed in managing a variety of medical issues, fostering long-term relationships with patients and families, and offering patient-centered care, with opportunities for precepting students. Faculty share responsibilities for night, weekend, and vacation coverage with other members of the group practice and will participate in decisions regarding the practice operation. This position will be housed on Anschutz campus within Children's Hospital Colorado Adult Special Care Clinic. The position's primary appointment will be with General Internal Medicine (.5 FTE), with a secondary appointment with Pediatrics (.5 FTE).
The Adult Special Care Clinic is seeking a physician (MD/DO) to provide primary care to adults with intellectual and developmental disabilities and complex medical needs. The Adult Special Care Clinic is an established and growing primary care home housed at Children's Hospital, where we provide primary care and support healthy transitions to adult specialty and inpatient care. Our patient population is limited to people 18 years of age and older with intellectual or developmental disabilities whose medical complexity requires coordination with multiple specialists and home care agencies. The clinician-educator will build and care for their own patient panel, ensuring patients receive excellent and individualized primary care in addition to coordinating communication and transition between both pediatric and adult subspecialists. In addition to caring for our core population, our providers are also expected to provide urgent and primary care for medically complex pediatric patients of all ages in need, in partnership with the pediatric Special Care Clinic, although ASCC providers do not carry a pediatric patient panel.
The clinician-educator will have opportunity for academic promotion judged on criteria of demonstrated excellence as a clinician/educator/scholar. Clinician-educators are encouraged to collaborate with other faculty in clinical research projects.
Key Responsibilities:
- Providing primary care, including urgent visits, to medically complex adult patients in the outpatient setting. This includes physically seeing patients in clinic, as well as assisting with phone triage of patients at other times.
- Partnering with our adult colleagues to provide primary care, including urgent visits, to a limited number of medically complex pediatric patients in the outpatient setting.
- Phone triage responsibility will include participating in a regular rotation of covering Provider of the Day, which will either be assigned during clinical or administrative time, depending on need. Daytime urgent visits outside of scheduled clinic sessions will sometimes be required in order to provide effective primary care.
- As a .05 cFTE, the clinical expectation is 4 clinic sessions/week in addition to 1 admin sessions/week. Each clinic session is four hours in length. This equates to 184 sessions/year. Program leadership holds the right to determine when the admin sessions can be taken based on clinic need.
- Participation in nighttime/weekend call rotation (shared with adult clinic) to manage patient/family phone calls outside of business hours.
- Coordinating care with a diverse care team that includes adult subspecialists and community providers.
- Working closely with our interdisciplinary team, including nurse and MA case management, family navigators, clinic nurses, dieticians, and social work to ensure that community services are available to patients.
- Engaging with inpatient care teams at University of Colorado Hospital to ensure smooth inpatient-outpatient transitions of care. This may, depending on interest, include visiting the patient in the hospital or telephone/electronic communication/coordination with inpatient care team.
- Precepting learners (potentially FNP, medical students, and residents in pediatrics, medicine/pediatrics, internal medicine, and family medicine) with special interest in caring for medically complex patients.
- Participating in ongoing quality improvement efforts to improve system-wide care for adults with intellectual and developmental disabilities.
Work Location: Onsite - This role is expected to work onsite and is located in Aurora, CO.
Qualifications:
Minimum Qualifications: Applicants must meet minimum qualifications at the time of hire.
This is an open rank position and could be categorized as Assistant Professor or Associate Professor based on experience and qualifications as indicated below:
Assistant Professor:
- MD or DO degree
- Board-certified or board-eligible in Med-Peds
- Valid CO medical license
- DEA Certification (or eligibility)
- 0-7 years of primary care experience
Associate Professor:
- MD or DO degree
- Board-certified or board-eligible in Med-Peds
- Valid CO medical license
- DEA Certification (or eligibility)
- 8 or more years of primary care experience
Preferred Qualifications:
- Demonstrated interest and experience in caring for adults with developmental disabilities.
- Proficiency in electronic medical records (EMR) systems, preferably Epic.
- Experience or interest in education, research and faculty development.
- Interest or experience in academic medicine, teaching, or mentoring medical students
Anticipated Pay Range:
MD or DO:
Assistant Professor: $100,000 - $107,000 ($88,590 - 92,232)
Associate Professor: $109,500 - $117,000 ($109,500 - 117,000)
