Your Role on the Team
Position Summary:
Incumbent is responsible for high level screening, assessment and planning for an identified high risk group of patients primarily in the Interventional Radiology Department, but in other service areas as needed. Collaborates with the attending and resident staff, as well as nursing, ancillary, professional, and technical staff, to coordinate the delivery of high quality, individualized, cost-effective patient care. Utilizes UR criteria (Interqual) to appropriately screen patients for admission or discharge. Concurrently, 1.) intervenes to enhance patient quality outcomes, 2.) ensures clinical information in the medical record is clear and complete reflecting the patient's accurate illness , and 3.) works collaboratively with physicians to improve the overall quality of the clinical documentation and to establish accurate assignment of patient status orders (e.g., observation, inpatient). Maintains ongoing communication with the patient/family , medical staff, caregivers, and third party payers to set priorities, plan, organize and implement care that is goal directed toward quality outcomes.
What It Takes to be Successful
Required Qualifications:
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must possess complete understanding of acuity levels for specific patient population
- Must demonstrate customer service skills appropriate to the job
- Minimum 5 years of clinical experience
- Minimum 2 years of clinical experience working on a specialty unit (Med Surg, any ICU, ED, Mental Health)
- Familiarity with Utilization Review criteria
- Familiarity of Joint Commission, CMS, CDPH and CCS requirements
- Excellent written and verbal communication skills in English
- All external candidates must have a Bachelor of Science in Nursing
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications:
- Previous experience working as a RN in an emergency or trauma department
- Previous Case Management experience
- Knowledge of University and medical center organizations, policies, procedures and forms
- Bilingual skills
- BSN and/or Master's degree
- ACMA and/or CCM Certification
Licenses and Certifications:
- Registered Nurse
