Coordinate with information technology and quality departments to ensure quality measures are met and data is recorded accurately.

Utilize clinical protocols to refer patients for appropriate screenings and diagnostic testing

Maintain knowledge of current state, federal, and facility requirements to meet current healthcare goals related to patient
safety, reimbursement, and healthcare quality standards.

Work with a multidisciplinary team to develop and implement an up-to-date plan of care.

Assess, develop, and implement effective patient and family teaching to accurately assess readiness to learn.

Communicate with all members of the healthcare team on behalf of the patient, caregivers, outside entities that are included in the care of the patient

Assist the patient with referrals to the patient financial counselor when needed

Collaborate with the marketing and outreach departments to educate referring physicians and local communities about the facilities and services provided.

Establish quality improvement projects with residents and fellows.

Maintain a patient database and participate in patient recruitment for clinical research.

Required Education/Experience

• Graduate from an accredited school of nursing.
  
  
• Nurse Navigator experience

Required Certifications/Licenses

• Certified RN. (Must possess and maintain a current license or valid permit to practice in TX)
  
  
• Possess and maintain current BLS Certification.
