"Nurse Practitioner - DLD (IBD)"

Position Summary

The Nurse Practitioner (NP) in the Inflammatory Bowel Disease (IBD) Program provides advanced clinical care to patients with Crohn’s disease, ulcerative colitis, and other complex gastrointestinal conditions. Working collaboratively with physicians and the multidisciplinary care team, the NP ensures comprehensive, patient-centered care. This role includes direct patient care and coordination of services to optimize disease management, enhance quality of life, and support continuity of care.

Responsibilities

Clinical Care:

• Conduct comprehensive assessments, diagnoses, and management of IBD patients in both inpatient and outpatient settings.
• Perform follow-up visits, manage medication therapies, and monitor treatment responses.
• Coordinate and oversee infusion therapy, biologics, and immunosuppressive medications.
• Monitor for adverse treatment effects and escalate care when necessary.
• Document patient histories, physical exams, and clinical decisions in accordance with institutional and regulatory standards.

Care Coordination & Patient Education:

• Serve as a primary point of contact for IBD patients and their families, providing education on disease processes, treatment options, medication adherence, and lifestyle modifications.
• Facilitate communication among providers, patients, and external healthcare professionals.
• Coordinate multidisciplinary services, including nutrition, mental health, surgery, and social work, to ensure holistic care.
• Triage patient calls, MyConnect messages, and urgent clinical concerns.

Minimum Qualifications

• Master's degree in Nursing or equivalent education and experience, plus one to two years of related experience.
• Current New York State (NYS) NP license (or proof of application) and RN license, as well as board certification.
• Current Drug Enforcement Administration (DEA) registration.
• Excellent communication (written and verbal), interpersonal, and organizational skills.
• Availability for a full-time, five-day workweek.
• Strong clinical assessment and decision-making abilities.

Preferred Qualifications

• Bilingual proficiency in English and Spanish.
• Prior clinical experience in Gastroenterology, Endoscopy, or General Surgery.
• Experience with EPIC or similar electronic medical record (EMR) systems.

• Medical clearance and participation in the medical surveillance program
• Contact with patients and/or human research subjects
• Potential bloodborne pathogen exposure
• Successful completion of applicable compliance and systems training requirements

Salary Range: $120,000 - $150,000
