Minimum Qualifications
Bachelor's Degree in a health and human services field or five (5) years case management and/or lived experience. Degrees must be received from appropriately accredited institutions.
Essential Job Functions
The Outreach and Family Services Coordinator provides family navigation, case management, and outreach services to families of children with special needs. This position plays a key role in community engagement by facilitating parent-to-parent matches, facilitating support groups, and delivering educational workshops. Additionally, the role includes working in collaboration with the Growing Well Clinic to assist with family support services.
- Family Navigation & Case Management: Provide individualized support to families, connecting them to appropriate healthcare, educational, and community resources.
- Parent-to-Parent Support: Facilitate parent matches, coordinate Circle of Parents Support Groups, and provide guidance to caregivers navigating the challenges of raising a child with special needs.
- Educational Workshops & Outreach: Organize and assist in delivering Triple P seminars and training sessions for parents and caregivers, and other educational workshops as requested.
- Community Collaboration: Work with DSS, CDSA, public schools, and healthcare providers to strengthen partnerships and streamline services for families.
- Program Monitoring & Reporting: Maintain accurate records of family interactions, referrals, and program activities for reporting and compliance purposes.
Preferred Qualifications
Lived experience caring for child(ren) or youth with special health care needs.
Work Schedule/Hours: Flexible hours with some evenings being required. Up to 20 hrs./week.
