Position Summary/Department Summary
The Children's Hospital Primary Care Center (CHPCC) is a primary care practice located at the main campus for BCH. CHPCC provides well-child medical care, including routine immunizations, health education, and urgent care for acute illnesses. CHPCC has one of the largest pediatric practices in Boston. CHPCC families are diverse and experience a range of socioeconomic and/or medical complexity that our dedicated staff responds to. Our staff includes phone room personnel, front desk personnel, administrative staff, medical providers, nurses, clinical assistants, child life specialists, and staff dedicated to mental health/social needs (i.e. psychologists, social workers, patient navigators).
Key Responsibilities:
- Engage with patients and caregivers in-person or by phone to assess health-related social needs such as housing, transportation, food access, and financial support.
- Connect families with appropriate internal and community resources, including referrals and service navigation.
- Serve as a liaison by building and maintaining relationships with healthcare staff and community agencies to improve service access.
- Provide care coordination and support to help families overcome logistical barriers to receiving medical care.
- Document all interactions and interventions accurately in the electronic medical record (EMR) system.
- Collaborate with multidisciplinary teams, contribute to program development and quality improvement initiatives, and participate in regular supervision and team meetings.
Minimum Qualifications
Education: High School diploma/GED required, bachelor's degree in social work referred. Other language fluency preferred. 1 years of prior experience working in community agencies and/or a medical setting required.
