Pharmacist- Transitions of Care Team (Main Campus)
Position Summary
The Pharmacist on the Transitions of Care Team is responsible for supporting safe and effective medication use during key transition points in a patient's hospitalization. This role focuses on medication reconciliation, discharge prescription review, patient education, and interdisciplinary collaboration to improve medication safety and continuity of care for patients receiving specialized cancer treatment.

Key Responsibilities
- Conduct comprehensive medication reconciliation at admission and discharge.
- Review and verify discharge prescriptions in the Epic electronic health record in accordance with institutional policies and applicable state regulations.
- Provide individualized patient counseling and medication education to support safe medication use after discharge.
- Identify and resolve medication-related issues in collaboration with prescribers and other healthcare professionals.
- Coordinate care with Nursing, Medical Teams, Outpatient Pharmacy, Discharge Planning, Infectious Disease, Pain Service, Case Management, and other stakeholders.
- Support initiatives that promote medication safety, operational efficiency, and high-quality patient care.
- Participate in departmental quality improvement and patient safety activities as appropriate.
- Perform other duties as assigned.
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Effective communication, organizational, and interpersonal skills.
- Current New York State Pharmacist license.
Preferred Qualifications
- Experience in transitions of care, medication management, or medication reconciliation.
- Completion of a PGY-1 Pharmacy Residency.
- Prior experience in hospital, ambulatory, or specialty pharmacy practice.
- Experience with Epic and Omnicell systems.
Schedule: Day shift 8 am to 4 pm. Rotating weekends and holidays.
