Pharmacy Tech Senior / Inter / Assoc - Transitions of Care - Transplant and Meds to Beds
This Pharmacy Technician position will focus primarily on supporting the transplant care team through direct coordination of medication-related needs for transplant patients. The technician will work closely with the transplant interdisciplinary team and the Taubman Pharmacy staff to streamline the discharge process and ensure timely medication delivery.
Pharmacy Technician, Transitions of Care- Transplant and Meds to Beds duties include:
- Collaborate with the transplant clinical team and specialty patient financial counselors to identify upcoming discharges and proactively manage medication needs for solid organ transplant patients.
- Communicate with the Taubman Pharmacy team to ensure transplant discharge medications are filled, verified, and ready for patients on the day of discharge.
- Coordinate deliveries for transplant medications to ensure timely arrival and proper handoff to patients or care teams.
- Monitor and coordinate any changes to discharge plans within 2 hours of a scheduled delivery, ensuring timely updates to the Taubman pharmacy and delivery teams to prevent delays or errors.
- Assist with Meds to Beds technician staffing coverage as needed to support overall discharge operations. This may include processing, billing, and coordinating prescription deliveries for Taubman patients scheduled for discharge and assigned for Meds to Beds delivery.
- Provide backup coverage and support for other transitions of care pharmacy technicians as needed.
- Collect data to determine process and outcomes measures of program.
- Assist with problem solving and process improvements relevant to the program.
- Collaborate with nursing, social work, providers, pharmacists, and others to optimize patient care.
- Other duties and projects assigned by supervisor to facilitate safe and efficient medication-related transitions of care.
High school diploma or an equivalent combination of education and experience is necessary. Ability to follow oral and written instructions, prioritize with accuracy and dependability. Fluency in reading, writing and speaking English. Strong communication skills, professionalism, ability to work well with members of an interdisciplinary team, and ability to provide service excellence are also required. Ability to perform a wide range of physical activity: lifting, pulling, bending, manual dexterity movements. Ability to follow oral and written instructions, prioritize with accuracy and dependability: Fluency in reading, writing and speaking English.
Pharmacy Technician Associate:
- High School Diploma or equivalent combination of education and experience
- Valid Michigan Pharmacy Technician License on or before hire date, verifiable through the State of Michigan Department of Licensing and Regulatory Affairs
- Proficient communication, computer and basic mathematic skills are necessary.
- Must be able to prioritize with accuracy and dependability.
Pharmacy Technician Intermediate:
- High School Diploma or equivalent combination of education and experience.
- Valid Michigan Pharmacy Technician License on or before hire date, verifiable through the State of Michigan Department of Licensing and Regulatory Affairs.
- One or more years of relevant pharmacy practice or equivalent experience is necessary.
- Proficient communication, computer and basic mathematic skills are necessary.
- Must be able to prioritize with accuracy and dependability
Pharmacy Technician Senior:
- High School Diploma or equivalent combination of education and experience
- Valid Michigan Pharmacy Technician License on or before hire date, verifiable through the State of Michigan Department of Licensing and Regulatory Affairs
- Two or more years of relevant pharmacy practice or equivalent experience is necessary.
- Proficient communication, computer and basic mathematic skills are necessary.
- Must be able to prioritize with accuracy and dependability
At least one year technician experience in an outpatient pharmacy setting and completion of a Pharmacy Technician training program is highly desirable. Demonstrated record of excellence with direct patient interaction, collaboration with an interdisciplinary team, insurance claim adjudication, prior authorization facilitation, and prescription processing in a retail pharmacy setting is also highly desired.
The State of Michigan Department of Licensing and Regulatory Affairs (LARA) requires all Pharmacy Technicians to be licensed. The new regulations allow for a one-time temporary license for up to 1 year, during which time a Technician must successfully complete requirements to become certified and meet all other State of Michigan requirements for a Full Pharmacy Technician License. This is a hybrid, day shift position, Monday- Friday. This position will be required to work on-site at the Taubman Ambulatory Care Pharmacy at least one day per week, based on the scheduling needs of the Meds to Beds and transplant teams, in support of discharge coordination and in-person collaboration. This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
