This position will serve as a functional member of the Quality Improvement Team in implementation of program goals and objective. This role involves a systematic approach to monitoring, assessing and improving health care quality at SIU Springfield Center for Family Medicine. Primary functions will include review of charts/processes, gathering/collecting data, working reports, and assisting with the assessment and monitoring of quality metrics. This position will work closely with the Director of Quality and CMO and collaborate with physicians, residents and other members of the health care team and patients to promote the delivery of quality, efficient and cost-effective healthcare services in a patient-centered environment. This person will also over patient care coverage in clinic when necessary.

Direct Patient Care: 50%

Direct patient care duties will be under the direction of the site leadership including the medical director, nurse administrator and/or director of clinical operations.

  • In accordance with established nursing policies and procedures, is able to prepare and administer oral medications and intramuscular, subcutaneous, and intradermal injections, perform wound care, including dressing changes, splint and cast removal, suture and staple removal, perform diagnostic and screening tests, etc.
  • Performs audits of narcotics and other clinically related areas and reports discrepancies; assists in maintaining adequate inventory of supplies.
  • Identifies cessation of heartbeat or respiration, notifies appropriate medical and nursing personnel, initiates resuscitation (such as external cardiac massage and restoration of breathing) and assists physician with preparation of medication or other treatment as indicated.
  • Assists in the operation of commonly used types of respirators; explains function and purpose of equipment to patient; initiates therapy and observes effect on patient.
  • Draws blood through venous puncture.
  • Transcribes physician's instructions from order sheet to multiple forms using prescribed format.
  • Acts as group leader for ancillary nursing personnel, preparing assignments, guiding personnel in giving nursing care, holding group conferences, preparing nursing care plans; and participates in the training and education of residents, medical students and new employees.
  • Assists with scrub and circulating nurse duties in surgical or clinic procedures.
  • Participates in clinic team activities, such as huddles or quality initiatives to communicate and shares information with the health care team that will assist in both proactively addressing service needs and also assist in shoring up gaps in care.
  • Assists with the creation of processes for coordinating and incorporating the information and recommendations received from external providers into the patient's care management plan. Includes consults and insurance company patient information.
  • Under indirect supervision by a professional nurse, assumes charge responsibility for a clinical unit.
  • Performs duties listed for the lower level of this series as appropriate.
  • Performs other related duties as assigned.

Indirect Patient Care - 50%

  • Utilizes clinical licensure knowledge to review patients charts and provide patient outreach to assist in the improvement of coordination of care.
  • With assistance from the Director of Quality and Quality Specialist, coordinates Patient Centered Medical Home Recognition for multiple clinic sites, including annual reporting and transformation process.
  • Uses clinical licensure knowledge to assess and review clinical workflows and offer suggestions on transformation opportunities.
  • Reviews disease-specific and preventative maintenance reports and works lists, reviewing charts and contacting patients via phone and/or letter to notify patients in need of particular services. Assists patients with compliance of identified services and in overcoming barriers to gaps identified.
  • Utilizing standing orders, protocols and evidenced-based guidelines, works in a collaborative effort with the patient, family, physician, and other members of the health care team to engage in and promote identified services. Documents activities in the EHR.
  • Becomes familiar with clinical quality metrics and practice transformation opportunities in order to assist with the overall improvement of patient care.
  • With guidance, reviews and maintains knowledge of quality related practice transformation opportunities and guidelines, and ensures practices are meeting minimum standards and guidelines.
  • Works with support staff and informatics nurse to assist in identifying and implementing reports or processes that improve follow-up of care, such as laboratory and diagnostic testing, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
  • Assists Director of Quality and Quality Specialist in creating, trending and generating quality reports.
  • Attends Quality meetings, staff meetings and conferences as requested by the Director of Quality and CMO.
  • Works directly with the faculty/residents/staff toward improvement of identified processes.

1. Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.

2. Six months of nursing experience in a health care facility (such as a hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities).
