PS Medical Outreach Case Mgr
Responsible for the coordination of supportive services and prevention-focused patient care in a fast-paced multidisciplinary clinical setting. Provides HIV prevention navigation and case management services aimed at increasing access to and retention in HIV prevention services, including Pre-Exposure Prophylaxis (PrEP) and Non-Occupational Post-Exposure Prophylaxis (nPEP).
Serves as a primary point of contact for individuals seeking HIV prevention services and supports patients in accessing prevention-related medical care, medications, insurance coverage, pharmaceutical assistance programs, and community resources. Assists clients in overcoming barriers to prevention services and works collaboratively with patients to develop the health system literacy, self-advocacy skills, and resource knowledge necessary to successfully engage in ongoing prevention care.
Provides education, advocacy, support, and empowerment to clients as they navigate healthcare and social service systems. Promotes long-term engagement in prevention services through ongoing follow-up, care coordination, and linkage to supportive resources.
Serves as a liaison between the University of Utah HIV Prevention Program and community referral partners. Develops and maintains collaborative relationships with healthcare providers, public health agencies, community-based organizations, educational institutions, and other referral sources to strengthen access to HIV prevention services throughout Utah.
Supports outreach, volunteer engagement, prevention education, and community partnership activities designed to increase awareness of HIV prevention services and reduce barriers to care.
Responsibilities:
- Conducts comprehensive assessments of client needs related to HIV prevention services, including PrEP, nPEP, medication access, insurance coverage, healthcare navigation, and supportive service needs.
- Provides linkage to HIV prevention services and coordinates access to PrEP and nPEP care, including scheduling appointments, facilitating referrals, and assisting clients in navigating prevention-related healthcare systems.
- Assists clients in obtaining and maintaining access to PrEP and nPEP medications through insurance programs, pharmaceutical assistance programs, patient assistance programs, and other available funding sources.
- Provides ongoing follow-up and retention support to promote continued engagement in HIV prevention services and reduce barriers to medication adherence and preventive healthcare.
- Educates clients regarding HIV prevention strategies, PrEP, nPEP, sexual health, healthcare navigation, medication access, and available community resources.
- Provides information and referrals for community resources including healthcare services, insurance programs, transportation, behavioral health services, substance use treatment, housing assistance, and other supportive resources as appropriate.
- Develops and maintains relationships with healthcare providers, public health agencies, community-based organizations, educational institutions, and other referral partners to strengthen HIV prevention referral networks and improve access to care.
- Serves as a primary liaison to community referral sources and partner organizations, responding to questions, coordinating referrals, facilitating communication, and identifying opportunities for collaboration.
- Assists with outreach activities, prevention education initiatives, volunteer engagement efforts, community presentations, and events designed to increase awareness of HIV prevention services.
- Supports the recruitment, training, coordination, and engagement of community and university volunteers participating in HIV prevention activities and outreach efforts.
- Tracks referrals, linkage outcomes, retention measures, prevention program utilization, and other programmatic indicators to support quality improvement and program evaluation efforts.
- Provides timely documentation in databases and patients' medical records, including interactions, interventions, referrals, and community resource contacts.
- Regularly attends meetings with clinical teams, prevention staff, public health partners, and community organizations to support coordinated service delivery and prevention planning.
- Manages a caseload of prevention clients receiving navigation and support services.
