An RN or LPN who can sit with a case manager, read the chart, and tell the difference between a patient who needs home health and a patient who needs hospice — the hard conversations and decisions only clinical post acute experience can provide. We are looking for the person who will be our face in Baldwin County: Fairhope, Daphne, Spanish Fort, Foley, Gulf Shores, and the communities around them.
We are searching for someone who will
- Build and keep relationships with hospital case management, discharge planners, physician offices, skilled nursing facilities, assisted living, and community partners.
- Represent both home health and hospice honestly. Census matters. So does the referral we should not have taken.
- Do the field work: rounds, in-services, lunch-and-learns, and the follow-up when a plan changes overnight.
- Work with intake and clinical leadership so the handoff is clean and the patient actually starts care.
- Track activity and conversions, and tell us early when an account is going quiet or a competitor is in the room.
- Stay inside the lines. Anti-kickback, HIPAA, and our admission criteria are part of the job.
Qualifications
- A current Alabama RN or LPN (compact license acceptable).
- Home health, hospice, or post-acute clinical experience. Liaison or marketing experience helps. Clinical judgment is required.
- Existing Baldwin County relationships, or the drive to build them.
- Someone comfortable working independently and being the only Saad person in the room.
- A clear communicator and a valid driver’s license. This job lives in the car.
Compensation details: 65000-90000 Yearly Salary
