Job Summary/Basic Functions

Perform competent and compassionate care to patients using nursing skills, assessment and education based on knowledge of the nursing process. Coordinate patient care as a team leader practicing Patient-Centered Medical Home principles

Minimum Qualifications

Education: Certification or graduation from a four (4) year college or university

Licensure & Certifications: Current KY RN license

Job Duties:

• 65% - Monitor patient vitals; detect changes in symptoms, health, and/or pain. Perform duties in compliance with care management and standing clinic orders. - (Essential)
• 20% - Fill out and update patient documents and charts. - (Essential)
• 10% - Maintain a safe and clean working environment by complying with procedures, rules, and regulations - (Essential)
