Job Summary
The RN Nurse Educator is responsible for exemplifying the University's Mission, Vision, and Values providing outstanding customer service. This position is an integral part of an interprofessional team serving as a coordinator and educator of chronic disease awareness, prevention, and management. This position works closely with the Program Director, Lifestyle Change Programs, and various faculty and clinical staff to assist with coordination, implementation, training, and technical assistance of internal and external clinical programs and processes.
This position is responsible for engaging in collaborative efforts to increase acquisition of patient/participant referrals through EPIC and/or EPIC CareLink bi-directional referral system. The RN Nurse Educator is responsible for delivering comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care. This position will be responsible for assisting the Lifestyle Change Programs in coordination of prevention and self-management opportunities with internal and external clinics within six specified census tract areas assigned by Texas Department of State Health Services.
The RN Nurse Educator provides collaborative, comprehensive, and education conducive to behavior change and improved quality of life across the lifespan to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. This position requires the ability to collaborate and function effectively as a team member in a broad spectrum and make moderately complex decisions with limited supervision.
Key Responsibilities
- Integrate skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes and cardiovascular conditions into clinical practice.
- Serve as Quality Coordinator for Association of Diabetes Care & Education Specialist (ADCES) program.
- Develop and complete all data tasks including reports and assessments required by ADCES and Center protocol(s).
- Coordinate, develop, implement, train and provide technical assistance for internal and external clinical staff on utilization of EPIC and/or EPIC CareLink in efforts to promote awareness and increase acquisition of bi-directional referrals through EPIC and/or EPIC CareLink.
- Actively assist Lifestyle Change Program Manager and Director with reporting measures.
- Advocate for and communicate improved quality of care and outcomes for those living with and/or at risk for and affected by diabetes and cardiometabolic conditions.
- Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan.
- Utilize appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.
- Actively assist Director with collaborative implementation of programs and services.
- Actively participate in the quality improvement (QI) process and adapts practice/process based on QI findings.
- Apply business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).
- Collaborate and network with community and/or clinical stakeholders to increase implementation and on-going support of programs and services.
- Engage in lifelong learning and serve as a role model of professionalism.
- Contribute to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
- Participate in Center, Department, and School of Medicine meetings and events, serve on committees, and represent in community outreach efforts as appropriate.
- Other duties as assigned.
Required Qualifications
- Bachelor's degree in Nursing.
- Diabetes self-management education and support experience preferred.
Required License
- Valid Registered Nursing License.
