Senior Case Manager
Position Introduction
The College of Nursing is seeking a dedicated and experienced Senior Case Manager to join our Case Management Department supporting home and community-based waiver programs. This position plays a critical role in coordinating services for aging and disabled populations while helping individuals maintain independence and quality of life within their homes and communities.
The Senior Case Manager will manage a full-time caseload and serve as an experienced resource within the department, providing guidance, support, and leadership to team members while ensuring high-quality case management services are delivered in accordance with organizational standards and program requirements.
Required Education and Experience
- Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related discipline.
- 24 months experience in home and community-based services, case management, or related healthcare setting.
Required Certifications and Licenses
Certified Case Manager (CCM). The employee is allowed 1 year from hire date or promotion to Senior Case Manager to obtain the CCM certification.
Department Preferences
- One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population.
- Certified in Case Management
