Senior Pharmacy Technician (CPhT) - TOC
Scope of Position
The Medication Management / Transitions of Care (TOC) Pharmacy Technician provides a valuable patient-centered service by focusing on facilitating TOC, including obtaining and documenting a complete and accurate admission medication history and allergy information in the electronic medical record (EMR) as well as assisting with the discharge medication process. The TOC Pharmacy Technician completes the admission medication history during an inpatient admission, ambulatory encounter, pre-admission testing, surgery encounter, or within a procedural area under the supervision of a pharmacist, physician, or advance practice provider.
Position Summary
The TOC Pharmacy Technician is responsible for obtaining a complete and accurate admission or ambulatory medication history and allergy information. The TOC Pharmacy Technician will help to streamline and facilitate the discharge medication delivery process when applicable.
Minimum Qualifications
- High School graduate or equivalent
- Active OH Board of Pharmacy Certified Pharmacy Technician registration, PTCB/NHA certification
- 2 years of relevant experience required. 4-6 years of relevant health care experience working as a Pharmacy Technician, with direct patient care preferred.
- Submit all required materials for registration as a Registered Pharmacy Technician to the OH Board of Pharmacy 10 business days before the official start date.
- Computer skills required in accordance with the requirements for data entry.
- Knowledge of medication names (both brand and generic) and their use, doses, routes of administration, and frequency at the time of hire or within ninety days of hire.
On-going: Obtain PTCB/NHA certification and OH Board of Pharmacy Certified Registered Technician status within first 270 days of employment and maintained for continued employment.
