Position Summary

In connection with the UNC Addiction Medicine Program, the Social Work Practitioner will assist with patient transitions both in the inpatient and outpatient settings, to include engagement with hospitals, Emergency Departments, Substance Use treatment programs and community providers. The Social Work Practitioner will assess case management needs and assist, as needed. This position will be responsible for assessment, planning and coordination of treatment services provided by the UNC Addiction Medicine Program, in addition to updating the treatment plan to meet a patient’s changing needs.

Responsibilities are listed below and include services provided both for the inpatient and outpatient settings:

• Provide Screening and Brief Intervention, including a Change Assessment and interest in treatment.
• Assist with the patient’s transition from the ED/hospital to UNC STAR Clinic and/or other requested outpatient treatment options available.
• Assist with patient’s intake assessment and treatment planning.
• Assess the patient’s case management needs at transition and during treatment and provide resources, as needed.
• Assist with patient’s transition to the appropriate community resources.
• Work collaboratively providing support and guidance with Peer Support Specialists both on the team and in the community at large.
• Attend organizational meetings and contribute to planning and workflow revision.
• Contribute to various UNC AMP associated projects, as needed.

Minimum Education and Experience Requirements

Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.

Management Preferences

Ability to drive 29-foot mobile medical vehicle (CDL not required).
