Social Worker
Position Description
Employees in this classification are responsible for providing social services to patients and their families by assisting them with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Persons in this position work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem. They also serve as a resource referral service in advising patients and their families about available community resources.
Major/Essential Functions
- Assess patient psychosocial needs and identify barriers to care.
- Provide counseling, crisis intervention, and supportive services to patients and families.
- Coordinate referrals and connect patients with community resources and support services.
- Collaborate with providers and care teams to develop and implement care plans.
- Investigate, document, and report suspected abuse, neglect, or exploitation; coordinate with CPS, APS, and other protective agencies as required.
- Assist patients with care transitions, discharge planning, and access to healthcare services.
- Advocate for patients and families to promote safety, well-being, and continuity of care.
- Maintain accurate documentation and comply with regulatory and professional standards.
- Participate in quality improvement, program development, and special projects.
- Provide education and training to staff, students, residents, and community partners.
- Represent the department on committees, workgroups, and community initiatives.
- Assist with development and maintenance of policies, procedures, and resource materials.
- Participate in emergency response and organizational preparedness activities as assigned.
- Attend professional development and continuing education activities to maintain licensure and competency.
