Sr Community Outreach Counselor (Campus Title Student Wellness & Case Management Coordinator) - Mount Wachusett Community College
General Statement of Duties:
The Student Wellness & Case Management Coordinator provides goal-oriented, strengths-based support to students experiencing academic, behavioral health, and basic needs challenges. This role serves as a key member of MWCC's CARE (Community Assistance and Risk Evaluation) Team, helping coordinate follow-up, track student progress, and ensure that at-risk students are meaningfully connected to supports both on campus and in the community.
The Coordinator complements the work of Counseling & Wellness by offering non-clinical case management, wellness outreach, and resource navigation. This includes helping students understand college processes, advocating when appropriate, and developing individualized action plans that promote persistence, safety, and holistic well-being.
Responsibilities:
CARE Team Support
- Serve as a primary point of coordination for students referred to the CARE Team due to behavioral, academic, or wellness concerns.
- Provide timely follow-up with students, gather relevant information, and communicate updates to CARE Team members.
- Track referral status, interventions, outcomes, and ongoing concerns using approved systems.
- Participate in CARE meetings, contributing observations and recommendations aligned with best practices in student support and risk evaluation.
Case Management & Student Support
- Offer non-clinical, short-term case management, including assessment, goal-setting, crisis-adjacent support, and resource coordination.
- Help students navigate college policies, academic procedures, and barriers affecting education (i.e absences, withdrawals, accommodations).
- Coordinate referrals to internal and external services such as food access, housing support, transportation resources, legal advocacy, medical care, disability services, and behavioral health providers.
- Provide warm hand-offs to community services including CSO, CHL, YWCA, recovery resources, and other partners.
- Maintain detailed, timely documentation of all interactions, referrals, and case progress.
- Support Counseling & Wellness outreach initiatives, including mental health awareness campaigns, resource fairs, and prevention programming.
- Assist in implementing workshops related to stress management, coping skills, self-care, and help-seeking behaviors.
- Help maintain the visibility and accessibility of wellness resources
- Work with students presenting with concerns such as:
- Academic Concerns (attendance issues, falling behind, missing work)
- Basic Needs & Financial Concerns (food insecurity, housing, transportation)
- Distressed Behavior (emotional dysregulation, overwhelm, worry)
- Disruptive Behavior (interfering with class or community operations)
- Dangerous Behavior (non-emergency concerns impacting safety)
- General Concerns (visible shifts in behavior, appearance, or functioning)
- Missing Students (sudden disengagement or disappearances from class)
Collaboration & Professionalism
- Work closely with the Senior Mental Health Counselor for supervision, guidance, and case review. Collaborate with the Wellness Outreach Coordinator on programs and events.
- Maintain strict adherence to FERPA, privacy expectations, and ethical standards.
- Build strong working relationships with faculty, advisors, Campus Police, Basic Needs services, and external community agencies.

Job Requirements:
Minimum Qualifications:
- Master's degree in Counseling, Education, Rehabilitation, Vocational Assessment, or closely related field;
- Two (2) years experience and/or training that includes career counseling, academic advising, project administration, or grant coordination; or
- An equivalent combination of education, training, and experience.
Desired Qualifications:
- Experience in higher education, case management, care coordination, or student support roles.
- Experience with trauma-informed approaches and behavioral health systems.
- Familiarity with community resources related to housing, food access, financial aid, crisis services, and healthcare navigation.
- Experience working with individuals experiencing behavioral health, academic, or social-emotional challenges.
- Demonstrated ability to work with diverse populations through an equity-centered lens.
- Strong communication, problem-solving, and organizational skills.
