Your Role on the Team
Position Summary:
The Supervisor, Case Management is responsible for the direct frontline supervision, daily workflow coordination, and operational oversight of case management coworkers within the assigned facility or service area. The incumbent is accountable for ensuring the timely and effective delivery of case management services in alignment with departmental policies, clinical standards, and all applicable regulatory and accreditation requirements, including The Joint Commission, Centers for Medicare and Medicaid Services, and applicable state and federal regulations. This role serves as the primary operational resource for case management coworkers during the assigned shift or designated area of coverage, and is responsible for the appropriate assignment of coworkers based on demonstrated competency, patient care needs, and operational demand. The Supervisor supports the onboarding, orientation, and ongoing training of case management coworkers, conducts competency monitoring, delivers real-time coaching and feedback, and initiates corrective action as warranted in alignment with organizational policy. The incumbent identifies and resolves operational, staffing, and patient care issues in a timely manner, escalates complex concerns to the Manager, and actively participates in quality, safety, and performance improvement activities. This position exercises independent judgment in managing the day-to-day supervision of assigned case management coworkers and the operational activities of the designated units, areas, or shifts.

Required Qualifications:
- Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
- Must possess complete understanding of acuity levels for specific patient population
- Must demonstrate customer service skills appropriate to the job
- Must be willing to work a flexible schedule, including weekends, and on call after hours as needed
- Minimum 3 years of Case Management experience
- Familiarity of Joint Commission, CMS, CDPH and CCS requirements
- Excellent written and verbal communication skills in English
- Demonstrated leadership, interpersonal skills, and ability to communicate with varied levels of customer audiences
- Demonstrated leadership skills/experience
- Demonstrated experience and ability to build effective working relationships and to represent the department effectively in order to accomplish goals
- Current California RN license
- Computer literacy and proficiency
- Bachelor's degree in Nursing or other relevant healthcare profession, or a combination of comparable clinical education and / or experience.
- All external candidates must have a Bachelor of Science in Nursing
- Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
- Ability to facilitate and lead interdisciplinary rounds
- Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications:
- Knowledge of University policies and procedures
- Knowledge of California Children's Services benefits authorization process (CCS)
- CCM, ACM or Case Management credentialing
- Bilingual skills
Licenses and Certifications:
- Registered Nurse
