Job Summary Xavier University of Louisiana is accepting applications for a PRN Telehealth pharmacist to assist the full time Telehealth pharmacist. collected across 8-Telehealth sessions per patient over 24 months. Additional duties will include data formatting, analysis and presentation of research findings in community and scientific forums. AA/EOE Additional responsibilities include: Adheres to XUTC policies/procedures related to HIPAA and security of patient data. Maintains compliance with all laws pertaining to the duties of a licensed, registered pharmacist. Provides instruction/education and feedback, as necessary, to student learners, residents, and fellows. Maintains necessary and appropriate records/documentation regarding services rendered. Ensures the safe and effective use of medications. Expected hours per month: 5-20 hours per months. Minimum Qualifications: PharmD Pharmacy Licensure in Louisiana PGY-1 Residency or 3 years related work experience Research Experience related to community engagement and pharmacy outcomes Preferred Qualifications: Fluency in additional language: Spanish or Arabic At least 1 year of work experience related to Telehealth OR At least 1 year of work experience related to Medication Therapy Management or Chronic Care Management OR At least 1 year of work experience related to Ambulatory Care Pharmacy Approved HIPAA training completed within the last year.
