Essential Job Duties
- Call patients regarding medication adherence
- Uncover challenges related to medication adherence
- Provide patients with suggestions and resources on how to improve medication adherence
- Contact pharmacies, physicians, and medical groups as needed to help improve overall care of the patient related to medication adherence
- Document call success rates and escalations within the provided excel template
- Document successful calls within Practice Fusion EHR
- Sign off on one’s own documented calls within Practice Fusion
- Attend training sessions and call center meetings
