The Pain That Doesn't Stay Put
Pancreatitis is inflammation of the pancreas, and its most reliable warning sign is pain that refuses to sit still. The ache usually begins in the upper middle or left side of the belly, then travels straight through to the back like a belt being tightened from the inside. It gets worse after food, worse when you lie flat, and it doesn't improve with the antacid tablets people understandably reach for first. That last part is worth repeating: the problem is not stomach acid, so heartburn remedies won't touch it.
Nobody tells you that the word “indigestion” is often just a placeholder for pain we have not yet explained. A lot of people spend an evening double-checking what they ate before they go to an emergency department at 2 a.m. with a lipase level several times the upper reference limit. The delay is human. The pancreas, for all its drama, is not an organ most of us think about until it gets angry.
The Organ Behind the Drama
Think of the pancreas as a combined dishwasher and payroll office for digestion. It lives behind the stomach, a roughly six-inch, tadpole-shaped gland that nobody photographs for fitness magazines. It makes digestive enzymes, including lipase for fat and amylase for carbohydrate, and it releases insulin and glucagon, the hormones that keep blood glucose from crashing or soaring.
When the organ is healthy, those enzymes stay inactive until they reach the small intestine. In pancreatitis, they switch on too early, inside the pancreas itself. The result is a bit like running the dishwasher with the door open: the cleaning agents start breaking down the machine. That is why acute pancreatitis can make a person profoundly unwell quickly, and why repeated attacks leave permanent scarring.
Signs and Symptoms You Should Not Explain Away
Acute pancreatitis typically announces itself with pain in the upper middle or left abdomen that bores through to the back. Nausea and vomiting are common, as is a low-grade fever and a fast pulse. The belly may be tender to the touch, and some people find relief only by curling forward or drawing their knees up. If a gallstone has blocked the bile duct, the skin or whites of the eyes may turn yellow.
Chronic pancreatitis can look different. The pain may be less dramatic but recurrent, often after meals, and it can last for hours. Over time, people may lose weight without trying, pass stools that are greasy or difficult to flush, and develop diabetes because the insulin-producing cells have been damaged. Some people with chronic disease have little pain until the organ has lost a lot of function, which makes routine medical attention far more important than it feels.

Most people with a mild attack improve within about a week if they get fluids and supportive care. A severe attack can put someone in an intensive care unit, and the pancreas can develop fluid collections, infection, or dead tissue that becomes infected.
Chronic pancreatitis is a progressive disease. Repeated or ongoing inflammation gradually replaces working gland with scar tissue. The digestive and hormonal jobs of the pancreas decline. For many people, alcohol is the dominant cause of chronic disease; for acute disease, gallstones are the leading cause in many high-income countries. Smoking makes both worse, and mixing smoking with heavy drinking is a particularly efficient way to speed up damage.

What Actually Causes It
Gallstones are the most common cause of acute pancreatitis in many high-income countries, according to the National Institute of Diabetes and Digestive and Kidney Diseases. Heavy alcohol use is the leading cause of chronic pancreatitis. High triglycerides, certain medicines, abdominal injury, and a procedure called endoscopic retrograde cholangiopancreatography (ERCP) can also trigger acute attacks. In younger people with unexplained recurrent pancreatitis, genetic causes are often part of the story.
One nuance matters for daily life. A fatty meal does not cause pancreatitis in a healthy person. If triglycerides are already dangerously high, however, alcohol or a very fatty meal can be the final push. That is why doctors chase down the cause after a first attack rather than simply treating the pain and sending you home.
How the Diagnosis Gets Made
Clinicians diagnose acute pancreatitis using a short checklist. At least two findings need to be present: the pain pattern matches, the lipase or amylase level is more than three times the upper reference limit, and in severe or uncertain cases imaging shows inflammation. Lipase has become the preferred blood test because it is more sensitive and specific to the pancreas than amylase, which can rise for other reasons. An ultrasound of the gallbladder usually follows to look for stones, while a CT scan is often reserved for severe cases or when the diagnosis is unclear. Magnetic resonance cholangiopancreatography (MRCP) and endoscopic ultrasound (EUS) can examine the ducts when the cause stays hidden.
In chronic pancreatitis, no single blood test makes the diagnosis. Doctors combine imaging, a history of recurrent pain, and signs of lost function such as diabetes or difficulty digesting fat. Treatment: Fluids, Food, and a Surprising Amount of Restraint
For years, the habit was to starve the pancreas by keeping people from eating. That has changed. Starving the pancreas made intuitive sense — stop running the broken dishwasher. The trouble is that the whole kitchen still needs clean plates, and the rest of the body needs nutrition. In mild acute pancreatitis, current practice is to start eating again as soon as the person can tolerate it, usually within a day or two. Fluid through a drip remains the cornerstone of early care, and pain relief is given promptly. Antibiotics are not routine unless there is a confirmed infection, because prophylactic antibiotics do not improve outcomes and can cause harm. When gallstones caused the attack, the gallbladder is often removed within days or a few weeks to cut the chance of a repeat.
Severe acute pancreatitis is managed more intensively. People may need nutrition through a tube into the small intestine, drainage of infected fluid collections, or support in an intensive care unit. That shift matters because it reduces length of stay and gets people back to ordinary life faster.
For chronic pancreatitis, the treatment stack looks different. Stopping alcohol and smoking is the most effective way to slow progression, though it is rarely the easiest. Pancreatic enzyme replacement therapy (PERT), taken with meals and snacks, restores digestion and helps with weight and greasy stools. Pain may require a combination of medication, nerve blocks, and sometimes surgery. A small number of people with severe, unresponsive pain are considered for total pancreatectomy with islet autotransplantation, a procedure that removes the pancreas and returns the patient's own insulin-producing cells to the liver.
What Research Is Changing About Care
Some of the most useful changes in pancreatitis care have been about doing less, not more. For years, the instinct was to give large volumes of intravenous fluid; newer evidence suggests that overhydration can worsen breathing and kidney function, so teams now aim for careful replacement and frequent reassessment instead of an open tap. Feeding has also moved earlier. In many cases, eating as soon as it is tolerated protects the gut and reduces complications compared with prolonged bowel rest, even if the old habit was to starve the pancreas.
There is growing interest in genetic testing when acute pancreatitis recurs without an obvious cause. Variants in genes such as PRSS1 and SPINK1, along with CFTR variants linked to cystic fibrosis, explain some early-onset and familial cases, including people who never drank heavily. For now, the interventions with the strongest evidence are wonderfully dull: stop smoking, stop heavy drinking, and remove the gallbladder when stones are the cause.

When to Go to the Emergency Department
Severe constant pain in the upper belly that goes through to the back is enough reason to seek emergency care, especially if you cannot keep down fluids, feel faint, or cannot find any position that helps. Fever, fast breathing, confusion, yellow skin, or a racing pulse alongside that pain should not wait for a GP appointment. Acute pancreatitis can worsen quickly, and early fluids and monitoring are genuinely protective.
If you have had one attack, the most important follow-up question is why it happened. That answer, not the pain medicine alone, is what changes the odds. For most people, the cause is findable, and a plan built around the cause is far more useful than a list of foods to avoid forever. The pancreas doesn't care how busy your week is; it will not reschedule.
