David Jacobs, PharmD, PhD, is an Associate Professor in the Department of Pharmacy Practice at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences. He earned his PharmD in 2011 and PhD in Epidemiology in 2018 from the University at Buffalo School of Public Health and Health Professions, with a dissertation on relationships between bacteria, exacerbations, and hospitalizations in chronic obstructive pulmonary disease. Jacobs completed a PGY-1 Pharmacy Practice Residency at the University of Toledo Medical Center, a PGY-2 Infectious Diseases Residency at the University of Houston College of Pharmacy, and a Clinical Research Fellowship in Pharmacoepidemiology at the UB School of Pharmacy and Pharmaceutical Sciences. He began as a part-time Clinical Assistant Professor in the Department of Pharmacy Practice in 2014 while pursuing his PhD and was appointed full-time Assistant Professor in 2018 before promotion to Associate Professor.
Jacobs conducts pharmaceutical health services and outcomes research, pharmacoepidemiology, comparative effectiveness research, and implementation science, focusing on medication safety, deprescribing in older adults, transitions of care, and social determinants of health in community pharmacy. His highly cited publications include "Early hospital readmissions after an acute exacerbation of chronic obstructive pulmonary disease in the Nationwide Readmissions Database" (2018), "Proton pump inhibitors and risk of acute and chronic kidney disease: a retrospective cohort study" (2019), "Potentially inappropriate medications are associated with increased healthcare utilization and costs" (2020), and "Clinical and economic effectiveness of a pharmacy and primary care collaborative transition of care program" (2025). Since 2018, he has authored 20 peer-reviewed articles, ranking in the top 10 percent of pharmacy practice scholars nationally. Jacobs received the 2023 University at Buffalo Exceptional Scholar Young Investigator Award, the 2025 Buffalo Translational Consortium Clinical Research Achievement Top Award, and the 2025 American Pharmacists Association Best Clinical Paper Award. He serves as Principal Investigator for grants from the Community Pharmacy Foundation and American Pharmacists Association Foundation, and Co-Investigator on awards from AHRQ, NIA, and NCATS. Jacobs is on the editorial board of JAPhA Practice Innovations and Assistant Director of Workforce Development at UB's Clinical and Translational Science Institute.
