Support patients, and whomever they define as family,
articulate health care goals, and provide information and referrals
for emotional/spiritual resources as needed.
Advocate for goal-concordant treatment and care for patients
and facilitate advance care planning.
Connect patients and families with hospice or other palliative
care programs (if appropriate).
Manage, organize, and update relevant data using database
applications.
Communicate effectively and efficiently with patients,
caregivers, and interdisciplinary care team members.
Participate in Quality Improvement projects.
Work Location:
Remote – this role is eligible to work remotely, but the employee
must be in the United States.
