This position will provide comprehensive care for older individuals in and across healthcare delivery systems.

This position is ON SITE only

Teaching 10%

  • Teach and evaluate medical students, residents, physician assistants and nurse practitioner students as they rotate through the educational program.
  • Conduct teaching rounds on individuals in selected skilled nursing facilities.
  • Provide teaching and modeling to fellows, residents and medical students on the principles, benefits and core concepts of interdisciplinary team care for frail geriatric patients with complex needs.
  • Work with geriatric interdisciplinary team members to define key outcomes expected from interdisciplinary care (i.e. Maintenance of function, decreased falls and delirium, fewer foleys and inappropriate medication, decreased LOS, fewer admissions in 30 days, increased patient and family satisfaction, etc.).
  • Assist with the development, evaluation and revision of the Springfield based Geriatric curriculum for residents and medical students.
  • Provide patient-centered quality care to older adults using evidenced based "best practices" to improve quality of care and health outcomes.
  • Attend Resident Core Conferences and assist in presenting as requested.

Service 85%

  • Assist older individuals, their families and caregivers to navigate transitions across healthcare delivery systems.
  • Provide medical management to skilled and custodial individuals in selected long-term care facilities.
  • Work in collaboration with the care coordination staff to manage care of frail individuals with complex health issues and co-morbidities.
  • Assist with determination of decision-making capacity, informed consent, palliative care, advance care planning, surrogacy, establishment of goals of care and end of life care management.
  • Conduct nursing home rounds with residents.
  • Identify health promotion needs, provide anticipatory guidance and counseling to address psychosocial, environmental, lifestyle and developmental issues of older individuals.
  • Evaluate and determine an individual's need for transition to a different level of care based on assessment of the individual's acuity, stability resources and need for assistance.
  • Plan and implement therapeutic interventions to stabilize older individuals, minimizing physical and psychological complications, maximizing the individual's health potential and optimize individual's health.
  • Assist individuals, families and caregivers to support or enhance the individual's sense of safety, autonomy, worth and dignity.
  • Assess individuals with complex health issues and co-morbidities, including the interaction with acute and chronic physical and mental health problems.
  • Provide support through effective communication and therapeutic relationships with older individuals, families and caregivers facing complex physical and/or psychosocial challenges.
  • Take part in quality and peer review activities.
  • Provide emergency services to patients as needed including those who are identified as having no primary care physician.
  • Attend faculty meetings and resident conferences.
  • Serve on the Center-wide Committees, as assigned.
  • Serve on school and department committees, as assigned.
  • Participate in community outreach projects.
  • Maintain competence in clinical practice using continuing education opportunities, professional conferences, and maintains licensure/certification.

Research 5%

  • Collaborate on research projects with Geriatrician and other faculty when possible.

Credentials to be Verified by Placement Officer

  • Sensitive to the needs of aging individuals.
  • Sensitive to the needs of underrepresented minority populations.
  • Interest in Geriatric and Palliative Medicine.
  • Certification as Family Nurse Practitioner, Adult Gerontology Primary Care Nurse Practitioner or Physician Assistant.
  • Licensed or eligible to be licensed in the State of Illinois as a Nurse Practitioner or Physician Assistant
