As a Clinical Pharmacist, you will be part of the Department of Pharmacy Hematology/Oncology team, as well as multidisciplinary teams within the University of Michigan Rogel Cancer Center. University of Michigan Health is ranked nationally for the care provided to both adult and pediatric cancer patients, as well as many other specialties. The cancer center mission is the conquest of cancer through innovation and collaboration. Current hematology/oncology clinical pharmacists provide exceptional pharmaceutical care on the individual and population level. The role of this pharmacist includes, but not limited to, the following:

  • Provide care to adult & pediatric patients at University of Michigan Health Hospitals, as an integral member of the multidisciplinary team.
  • Provide multidisciplinary rounding and fulfill direct patient care needs for adult and pediatric patients receiving SCT
  • Collaborate with providers on crafting and personalization of treatment plans for patients receiving stem cell transplants (SCT) and other forms of cellular therapy.
  • Manage graft-versus-host disease (GVHD) and other support care needs of the population
  • Guide incorporation of new treatments in a standardized evidence-based fashion, in conjunction with other members of the pharmacist team, including active involvement at tumor board discussions.
  • Coordinate care to promote optimal site of care to receive stem cell transplants and other forms of cellular therapy
  • Collaborate closely with ambulatory adult and pediatric lymphoma, myeloma, benign hematology, and BMT and cellular therapy teams to provide patient centered care and ensure seamless transition of care
  • Participate in SCT quality assurance initiatives, including protocol development, adverse event reporting and outcome analysis
  • Educate patients, families, house staff and learners on transplant pharmacotherapy
  • The stem cell transplant clinical pharmacist will actively participate in precepting learners and engaging in clinical research and quality improvement initiatives to advance evidence?based transplant car

Qualifications:

  • A Doctor of Pharmacy degree or an equivalent combination of education and experience.
  • Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable in LARA; or eligibility to become licensed within 90 days of employment start date.
  • A certificate of completion from an oncology residency, fellowship, or equivalent combination of education, training, and experience is necessary.
  • Experience in clinical pharmacy practice is necessary.
  • Board Certification in Oncology Pharmacy is desired, but not required

This position is full-time, working primarily Monday through Friday on day shift with periodic weekend rotation

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
